Research On The Rule Of Syndrome Differentiation And Medication Of Wang Ziyu In The Treatment Of Endometriosis Diseases Based On Data Mining

BackgroundEndometriosis and adenomyosis are collectively referred to as endometriosis,which is a common and refractory disease of women of childbearing age,in which Traditional Chinese Medicine(TCM)has advantages,and in which the gynecological experts of TCM have different understandings and characteristics of the medicine.Professor Wang Ziyu has cultivated the clinical,teaching and scientific research of gynecology of TCM for more than 70 years,and has achieved remarkable curative effect in the treatment of this disease,of which he has unique personal characteristics in law of syndrome differentiation and medication,which need to be studied and summarized urgently.ObjectiveTo discuss professor Wang's personal characteristics in law of syndrome differentiation and medication of endometriosis,and provide some clinical experience for fellow practitioners of TCM based on data mining and literature review.MethodsResearch on the law of syndrome differentiationA total of 150 cases of endometriosis treated by professor Wang from 2005 to 2015 were collected and analyzed in the way of descriptive analysis and bayesian network analysis by SPSS24.0 and Weka3.9.Professor Wang's academic successor participated in the whole process of statistics and discussion,which was referred to as man-machine combination mode.Study on the law of medicationA total of 502 prescriptions of endometriosis treated by professor Wang from 2005 to 2015 were collected and analyzed in the way of descriptive analysis,cluster analysis and association rule analysis by SPSS 24.0 and SPSS Modeler.ResultsResearch on the law of syndrome differentiationA total of 61 symptoms,10 types of tongue signs,and 9 types of pulse signs were selected in 150 cases,and the top 10 symptoms were lower abdomen pain,dark red menstruation,low blood clots,low back pain,breast tenderness during menstruation,menstrual cramps,lower menstrual flow,excessive menstrual flow,menstrual anal bulging,bright red menstruation.And common tongue signs were dark red and pale tongue,and common pulse signs were chord veins and fine veins.The two most common syndromes were kidney deficiency and blood stasis,qi stagnation and blood stasis.Blood stasis was a syndrome factor in all cases,and the syndrome factor combinations of kidney deficiency,liver stagnation,qi stagnation and blood stasis account for 71.33%.The recognition rate of bayesian networks was 94.67%in cold coagulation syndrome,90.67%in damp-heat syndrome,86%in qi stagnation syndrome,82.67%in liver depression syndrome,77.33%in kidney deficiency syndrome.4.The contribution degree of each symptom to syndromes was quantified,so as to distinguish the primary symptoms and the secondary symptoms,for example,the primary symptoms of cold coagulating were the dark red menstruation,menstrual nausea and vomiting,cold in the lower abdomen,cold sweat and cold limbs;the primary symptoms of damp-heat syndrome were leucorrhoea yellow and fat tongue;and the primary symptoms of Qi stagnation were menstrual breast tenderness,fat tongue and dark red tongue;the primary symptoms of liver stagnation were menstrual breast tenderness and string pulse;the primary symptoms of kidney deficiency were waist ache,string pulse and thin pulse.Study on the law of medication203 patients with endometriosis had a total of 493 visits,502 prescriptions of TCM,involving 131 types of Chinese medicine,a total frequency of 6100 times of use,35 types of high frequency(?40 times),and 13 types of Chinese patent medicines.Wang commonly used Danggui,Chishao,Danshen,Xiangfu,Yuanhu,Moyao,Ezhu,Baishao,Yimucao,Puhuang,and Wudan pill which was the only one Chinese patent medicine,and accounted for 50.49%of the patients.The results of cluster analysis showed that the distance between the three groups of Qingpi-Chenpi,Guizhi-Taoren,Puhuang-Wulingzhi was 1 and the relationship was the closest.And the high frequency Chinese medicines were divided into 10 categories by man-machine method,as C1:Qingpi,Chenpi;C2:Guizhi,Taoren,Paoshanjia,Sanqi;C3:Chaihu,Juhe;C4:Wuzhuyu,Xiaohuixiang,Rougui,Xixin,Wuyao;C5:Shanyao,Shanyao,Dangshen,Duzhong,Huangqi,Fuling;C6:Ezhu,Sanleng,Shijianchuan,Shijianchuan,Lizhihe;C7:Baishao,Shudi,Chuanxiong;C8:Xiangfu,Yimucao;C9:Puhuang,Wulingzhi,Yuanhu,Moyao;C10:Danggui,Chishao,Danshen.Association rule analysis showed that the support degree of the known herb pairs for Danggui-Chishao,Danggui-Baishao,Danggui-Chishao-Baishao,Ezhu-Sanleng,Puhuang-Wulingzhi and Yuanhu-Moyao,was above 25%,and the confidence degree was above 88%.The four herb pairs of Qingpi-Chenpi,Wuyao-Xiaohuixiang,Wuyao-Wuzhuyu,Xiangfu-Yimucao were consistent with the results of cluster analysis(C1,C4,C8);and showed Shanyurou-Shanyao-Shudi as one herb string.ConclusionsProfessor Wang took blood stasis blocking the uterus and Chong-Ren as the primary pathogenesis of endometriosis,focusing on the combination of disease differentiation and syndrome differentiation.Kidney deficiency and blood stasis,qi stagnation and blood stasis of endometriosis are the common clinical syndromes,while kidney deficiency,liver stagnation,qi stagnation and blood stasis are the common syndromes factors.Bayesian network has a high recognition rate of cold coagulation syndrome,damp-heat syndrome,qi stagnation syndrome,liver depression syndrome;and the primary symptoms are partly consistent with the clinical situation.In terms of treatment,Wang takes blood-activating and mass-eliminating as the primary treatment,among which blood-activating is the key,including tonifying kidney and activating blood,promoting qi to activate blood,invigorating qi and blood to activate blood,and warming the meridians and activating blood circulation;while mass-eliminating including breaking blood and eliminating mass,and softening and eliminating mass.The prescription has the characteristics of both attack and remedy,avoiding overattack,simplifying and effectivity,and stage treatment.Professor Wang commonly uses Danggui,Chishao,Danshen,Xiangfu,Yuanhu,Moyao,Ezhu,Baishao,Yimucao,Puhuang;and Wudan pill is a supplement to decoction;and commonly uses herb combinations including?Danggui-Chishao,?Danggui-Baishao,?Danggui-Chishao-Baishao,?Ezhu-Sanleng,?Puhuang-Wulingzhi,?Yuanhu-Moyao,?Xiangfu-Yimucao,?Wuyao-Xiaohuixiang,?Wuyao-Wuzhuyu,?Qingpi-Chenpi,(11)Shanyurou-Shanyao-Shudi,(12)Puhuang-Wulingzhi,(13)Guizhi-Taoren.
