The report of the 19 th National Congress pointed out that it is necessary to comprehensively build a multi-level social security system to cope with the problem of population aging,so that the old can be raised and treated.In the problem of population aging,the long-term care problem of disabled elders is particularly prominent.From the practical experience that has been carried out abroad,the construction of long-term care insurance(LTCI)system is an effective means to solve the problem of nursing for the disabled elders.China is also actively exploring the establishment of a LTCI system.In 2016,the General Office of the Ministry of Human Resources and Social Security issued the “Guiding Opinions on Piloting the LTCI System”,proposing to establish a LTCI system in 15 pilot cities across the country,officially opening up China.In 2019,the State Council's government work report once again clearly proposed to expand the LTCI system pilot,highlighting the importance of LTCI in the construction of a multi-level security system.From the pilot situation,the promotion of LTCI is generally from urban to rural areas.As a large-scale city in the central part of Wuhan,the status quo of population aging and the supply of long-term care services are in the middle of the country.The environment and foundation for the construction of LTCI system are typical.In this context,this paper focuses on the construction of LTCI system in Wuhan.Based on the reality of Wuhan and the experience of pilot cities,it is of great practical significance to study the ideas and countermeasures of building a LTCI system in Wuhan.This paper mainly adopts literature research method and comparative analysis method.The content structure of the article is arranged as follows: The first part combs and summarizes the literature on LTCI at home and abroad,defines related concepts,and determines five-dimensional analysis framework for LTCI based on Gilbert's social welfare policy theory,namely distribution basis,distribution content,service delivery,fund raising and treatment payment.The second part analyzes the realistic background of building a LTCI system in Wuhan.Under the background of the gradually expanding size of disabled elders,the lack of traditional nursing supply and the phenomenon of social hospitalization,the construction of LTCI system is an inevitable choice for Wuhan to cope with and solve the problem of nursing for disabled elders.Wuhan has a good policy and economic environment.The care hardware infrastructure and the corresponding affordability of residents provide the feasibility of building a LTCI system.The third part is based on the five-dimensional analysis framework,and deeply compares the institutional design of 15 pilot cities,and draws lessons from the experience of Wuhan.The fourth part combines the actual situation of Wuhan City,and proposes policy recommendations for the construction of LTCI system in Wuhan from two aspects: the main content of the system design and the system design supporting measures.In the main content of the system design: the insured objects gradually covered urban and rural residents,and the scope of protection gradually expanded to moderate and mild disability.The insurance payment was mainly service-oriented,the service delivery clearly defined the rights and responsibilities,the fund raising was carried out in multiple ways,the payment set proportion and quota,and the system design supporting measures were adopted: improve relevant legal policies,cultivate professional nursing talents,improve the service supply supporting system,formulate scientific disability assessment standards,and establish risk management prevention and control mechanisms.
