Objective:To observe whether dexmedetomidine nasal spray combined with intravenous application of remifentanil can effectively improve the recovery quality of general anesthesia and reduce adverse reactions during extubation.Methods:A total of 60 cases of patients with elective surgery of liver and gall and gastrointestinal ㎏ in our hospital were selected as the research object.According to the random number table method,they were divided into 3 groups: control group(C group),1.0 g/㎏ group(D1 group)and 2.0 g/㎏ group(D2 group),with 20 cases in each group.All the three groups were given the corresponding dose of Dex nasal spray 30 min before the end of surgery(dilute the drug to 2ml,spray 1ml on both sides of the nasal cavity,the control group was given the same amount of normal saline).When pneumoperitoneum was closed,the dose of Remifentanil was reduced to 3 g/(kg·h)and stopped after extubation.HR and MAP of patients at T0(entry),T1(at the end of the operation),T2(5min after extubation).The time of extubation time,Ramsay sedation score 10 min after extubation,postoperative cough reaction and postoperative agitation were recorded in the three groups.The frequency of adverse reactions in each group during extubation period was recorded,including hypertensive blood pressure,tachycardia,respiratory depression,hypoxemia,etc.Results:The general condition of the patients was not statistically significant(P > 0.05).There was no significant difference in mean arterial pressure and heart rate between the three groups at the end of operation(P > 0.05).At 5 minutes after extubation,the hemodynamic level of the three groups was compared pairwise.There was no significant difference between the D1 group and the D2 group(P > 0.05),while there were significant differences between the D1 and D2 groups and the control group(P <0.05).The hemodynamic level of patients in the D1 and D2 groups was more stable than that in the C group.Residual extubation time,sedation score,restlessness and other results were described as above,and no adverse reactions were found in the three groups.Conclusion:Above all,including 30 minutes before the end of surgery using 1.0 g / ㎏ dose right beauty holds the mi definite nasal combined intravenous application,fentanyl than simple applications,fentanyl anesthesia,in does not affect the extubation time get better wake up on the basis of quality,to revive the patients in general anesthesia period is more safe and comfortable,this scheme can be put in clinical,It is worthy of further study and popularization. |
