PurposeTo explore the correlation between serum uric acid(SUA)level and carpal tunnel syndrome(CTS)in patients with type 2 diabetes mellitus(T2DM),and to provide theoretical basis for the prevention and treatment of type 2 diabetes mellitus(T2DM).Method370 diabetic patients(205 males and 165 females)who underwent neuroelectrophysiological examination in the functional department of the First Affiliated Hospital of Dali University from December 2018 to December 2020 were selected as the research objects.All the selected subjects must meet the diagnostic criteria of type 2 diabetes,and Dantec keypoint was used According to the results and CTS diagnostic criteria,the subjects were divided into type 2 diabetes mellitus complicated with carpal tunnel syndrome group(CTS group: 124 males,118 females,mean age 58.32 ± 10.40 years),type 2 diabetes mellitus without carpal tunnel syndrome group(non CTS group: 81 males,47 females,mean age 58.73 ± 10.97years).At the same time,60 patients with non-type 2 diabetic carpal tunnel syndrome(NC group: 10 males and 50 females,mean age 51.83 ± 11.05 years)were collected for neuroelectrophysiological examination.All subjects had informed consent.Age,gender,duration of diabetes,occupation(manual workers and non manual workers)were collected.Height and weight were measured.Body mass index(BMI:weight / height 2)was calculated as general data.Serum uric acid(SUA),fasting plasma glucose(FPG),glycosylated hemoglobin(Hb A1c),triglycerides(TG),total cholesterol(TC)and low density lipoprotein cholesterol(LDL-C)were measured in all subjects Cholesterol(LDL-C),serum calcium(CA).The nerve conduction velocity(NCV)of the extremities was detected.If necessary,needle electrode electromyography was performed to observe whether there were fibrillation potential,positive sharp wave or spontaneous potential.Spss24.0 statistical software was used to analyze all the collected data.Result1.Comparison of clinical data of each group.Compared with NC group,age,course and BMI of non CTS group and CTS group were higher than that of NC group(P < 0.05).Compared with group Non-CTS,the duration of diabetes in group CTS was longer and BMI was higher than that in group CTS.The prevalence rate of manual workers was higher than that of non manual workers.The prevalence of male CTS in CTS and Non-CTS groups was higher than that in females,but the incidence rate of male in CTS group was lower than that in NonCTS group(the prevalence of female increased)(two < 0.05).Compared with NC group,the levels of SUA,FPG and TG in non CTS group and CTS group were higher than that in NC group,while Ca level was lower(P < 0.05).Compared with non CTS group,the levels of SUA,FPG and Hb A1 c in CTS group were higher than those in non CTS group,while Ca was lower than that of non CTS group(P < 0.05).2.Correlation analysis between SUA level and each index.SUA was positively correlated with diabetes duration and BMI.3.Relationship between SUA and T2 DM complicated with carpal tunnel syndrome.The SUA level in group CTS was higher than that in group Non-CTS and group NC(P<0.05).Compared with group Non-CTS,the incidence rate of hyperuricemia(Hyperuricemia,HUA)in CTS group was higher than that in group A.There was statistical difference between groups(f).The risk of T2 DM complicated with carpal tunnel syndrome in Hua group was 2.91 times higher than that in non Hua group.The median nerve abnormality rate and the severity of median nerve damage in Hua group were higher than those in non Hua Group(P < 0.05).4.Risk factors of carpal tunnel syndrome in patients with type 2 diabetes mellitus.Whether T2 DM patients complicated with carpal tunnel syndrome,SUA,FPG,Hb A1 c,CA,TG,BMI,age,gender,duration of diabetes,occupation(manual workers,non manual workers)were included in the regression equation for multivariate analysis.The results showed that SUA,FPG,Hb A1 c,CA,BMI,duration of diabetes,occupation could be the important predictors of type 2 diabetes complicated with carpal tunnel syndrome FPG,Hb A1 c,BMI,course of disease,occupation(manual workers)can be used as risk factors,while CA can be used as protective factors.Conclusion1.The incidence rate of CTS in T2 DM patients is high,which needs clinicians’ attention.2.SUA level in T2 DM patients with CTS was significantly higher than that in non CTS group,and the incidence of median nerve abnormality and severe median nerve damage in T2 DM patients with Hua was higher than that in non Hua group.SUA can be used as an independent risk factor for the occurrence and development of CTS in T2 DM patients.3.FPG,Hb A1 c,BMI,course of disease,occupation(manual labor)were positively correlated with the occurrence of CTS in T2 DM patients,which were risk factors for the occurrence of CTS in T2 DM patients,while Ca was negatively correlated,which could be used as protective factors for the occurrence of CTS in T2 DM patients.
