Objective: In this study,the concentration of monocyte chemoattractant protein1(MCP-1)in the gingival creval fluid extracted from patients with periodontitis and rheumatoid arthritis(RA)was determined by enzyme-linked immunosorbent assay.The periodontal and rheumatoid immune indexes were observed to analyze the role of MCP-1 in periodontal disease and RA.To explore the expression of MCP-1 in periodontitis and its relationship with periodontal and rheumatism indexes,and further explore the relationship between periodontitis and RA as well as the role of MCP-1,so as to provide a theoretical basis for the prevention and treatment of periodontitis and RA.Research Methods:1.Selection of subjects: According to the inclusion and exclusion criteria,24 patients with periodontitis and RA who were admitted to the Department of Stomatology and Rheumatology and Immunology of Liaoning Provincial People’s Hospital from June 2020 to November 2020 were selected as P+RA group [7 males,17 females,age(53.9±10.3)years].Twenty-two patients with simple periodontitis were assigned to P group [14 males,8 females,age(51.5±10.5)years old].Nineteen patients with simple RA were assigned to RA group(5 males,14 females,age(49.2±9.5)years);And 20 healthy subjects: group H [8 males,12 females,age(48.1±7.7)years old].There were 85 cases in 4 groups.2.Methods: Collect general data of 4 groups of subjects,including age,gender,body mass index(BMI),disease course of RA patients,stage of periodontitis patients,etc.The moisture absorption paper tip weighing method was used to extract gingival creval fluid samples from experimental subjects.The exponential teeth in each quadrant showing the largest bone loss on X-ray films were selected for collection.The samples were put into the numbered EP tube and placed in the-70° low temperature refrigerator for reserve.3.Periodontal index examination: The following periodontal examination items are performed for the epidemiological dental positions of the 4 groups of subjects:bleeding index(BI),pocket plaque depth(PPD),clinical atachment loss(CAL),simplified calculus index(CI-S),simplified soft debris index(DI-S),so as to record the corresponding data.The number of missing teeth(MT)was recorded.4.Rheumatism immune-related indicators: Anticycliccitrullinated peptide antibody(anti-CCP),rheumatoid factor(RF),C-reactive protein(CRP),and erythrocyte sedimentation rate(ESR)were measured in the two groups of RA patients.5.Enzyme-linked immunosoibent assay(ELISA)was used to detect the collected samples and obtain the data.6.SPSS23.0 software was used to analyze the correlation of the obtained data.Results:1.BI,PPD,DI-S,CI-S and MT in P+RA group and P group were significantly higher than those in RA group and H group(P < 0.05);Cal,Di-S and CI-S in P+RA group were significantly higher than those in P group(P < 0.05).PPD,BI and DI-S in RA group were significantly higher than those in H group(P < 0.05).2.Compared with RA group,various rheumatic immune indexes in P+RA group were higher than those in RA group(P < 0.05).3.The expression level of MCP-1 in gingival crevicular fluid of P+RA group was significantly higher than that of P and RA group(P < 0.05);The expression level of MCP-1 in gingival crevicular fluid of P+RA group and RA group was significantly higher than that of H group(P < 0.05).4.The expression level of MCP-1 in gingival crevicular fluid of P+RA group and P group was significantly positively correlated with periodontal indexes PPD and CAL(PPD: r= 0.23 P=0.034 CAL: r=0.615 P < 0.01).5.The expression level of MCP-1 in gingival crevicular fluid of P+RA group and RA group was significantly positively correlated with the expression of rheumatic indexes ESR and CRP(ESR: r=0.306 P=0.046 CRP: r=0.328 P=0.032).Conclusion:1.The expression level of MCP-1 in gingival crevicular fluid of patients with periodontitis with RA was significantly increased,which was positively correlated with CAL and PPD,suggesting a certain relationship between MCP-1 and the pathogenesis of periodontitis and RA.2.The clinical examination showed that PPD and CAL had the most severe damage in patients with periodontitis and rheumatoid arthritis,followed by patients with simple periodontitis and the healthy group,which indicated that rheumatoid arthritis might aggravate the progression of periodontitis.
