BackgroundAccording to the latest epidemiological survey,liver cirrhosis has become one of the14th leading causes of death in the world.Among the various causes of liver cirrhosis,different from the alcoholic disease in European and American countries,the main cause of liver cirrhosis in China is virus infection,especially hepatitis B virus.According to statistics,due to the increase of population and the change of age composition ratio in our country,the morbidity and mortality of liver cirrhosis have increased.Patients with chronic liver disease eventually die of liver cirrhosis and liver cancer due to the progress of the disease.At present,the treatment of the disease mainly adopts comprehensive programs such as anti-virus,prevention and treatment of complications,and improvement of liver function[1].In cirrhosis,various pathological mechanisms lead to intestinal flora disorder and small intestinal bacterial overgrowth With the continuous research on the mechanism of"gut liver axis",we know that SIBO can further aggravate the condition of liver cirrhosis.At the same time,most patients with SIBO will have gastrointestinal symptoms such as abdominal pain,abdominal distension,and changes in stool habits,thus affecting the quality of life.Studies have shown that low fermentable oligosaccharides,disaccharides,monosaccharides,and Polyols(FODMAPs)diet can further alleviate gastrointestinal symptoms and improve the quality of life of patients on the basis of antibiotic treatment[2].ObjectiveTo investigate the effects of low fermentable oligosaccharide,disaccharide,monosaccharide and polyol(FODMAPs)diet on gastrointestinal symptoms of patients with liver cirrhosis and intestinal bacterial overgrowth(SIBO)positive.MethodsThe subjects were 73 patients with gastrointestinal symptoms and positive hydrogen breath test of hepatitis B cirrhosis in Zhengzhou people’s Hospital in 2019.They were divided into observation group(n=37)and control group(n=36).During the 6-week trial period,on the basis of maintaining the original treatment,the observation group strictly implemented the low FODMAP diet,while the control group maintained the original diet.The changes of IBS symptom scale(IBS-SSS),the Mos 36-item Short Form Health Survey(SF-36),hydrogen production value(PH2)and grip strength(GS)were compared between the two groups.ResultsAfter 6 weeks of diet intervention,the IBS-SSS score of the observation group significantly lower than that of the control group(86.77±67.82vs130.74±73.97)and the difference is statistically significant(t=2.599,P<0.05);and the SF-36 score of the observation group significantly higher than that of the control group(62.89±4.69vs73.47±4.12)and the difference is statistically significant(t=10.035,P<0.001);The PH2 of the observation group mildly lower than that of the control group in the same period(39.83±15.94vs48.66±16.13),and the difference is statistically significant(t=2.303,P<0.05);but there was no significant difference in GS between the observation group and the control group(22.97±8.79vs23.06±9.13)(t=0.042,P>0.05).ConclusionLow FODMAP diet effectively improve gastrointestinal symptoms in patients with liver cirrhosis and SIBO positive in a short time.
