Study On The Correlation Between Serum CTRP3,CTRP15 Levels And Insulin Resistance In Patients With Polycystic Ovary Syndrome

Abstract/Summary:
Objective: To determine the serum levels of C1q/tumor necrosis factor-related protein 3(CTRP3)and C1q/tumor necrosis factor-related protein 15(CTRP15),to explore the correlation between CTRP3,CTRP15 and insulin resistance(IR)in PCOS patients.Methods: A total of 105 newly diagnosed PCOS patients were selected in the Affiliated Hospital of Zunyi Medical University from July 2017 to July 2020.Another 75 age-matched physical examination women were selected during the same time.All the subjects underwent Hyperinsulinemic-euglycemic clamp(HEC)to measure glucose metabolism rate(M value).They were divided into the following four groups according to whether companied by IR:(1)Normal control group(NC group,n=40),(2)insulin resistance group(IR group,n=35),(3)PCOS without insulin resistance group(PCOS+NIR,n=50),(4)PCOS with insulin resistance group(PCOS+IR,n=55).Serum levels of fasting plasma glucose(FPG),glycosylated hemoglobin(HbAlc),triglyceride(TG),total cholesterol(TC),high density lipoprotein-cholesterol(HDL-C),low density lipoprotein-cholesterol(LDL-C),alanine aminotransferase(ALT),aspertate aminotransferase(AST),fasting insulin(FINS),estradiol(E2),progesterone(P),follicle-stimulating hormone(FSH),luteinzing hormone(LH),prolactin(PRL),sex hormone binding globulin(SHBG),testo-sterone(TEST),dehydroepiandrosterone(DHEAS),CTRP3 and CTRP15 were determined.Body mass index(BMI),homeostasis model assessment of insulin resistance(HOMA-IR)and free androgen index(FAI)were calculated.The levels of CTRP3 and CTRP15 in each group were compared,the influencing factors of CTRP3,CTRP15 and their relationship with IR were analyzed.Results: Compared with NC group,BMI,WHR,SBP,DBP,FPG,TG,TC,LDL-C,FINS,HOMA-IR were significantly increased and HDL-C,CTRP3 decreased in IR group(P<0.05),BMI,WHR,FPG,HbA1 c,TG,TC,LDL-C,FINS,HOMA-IR,TEST were significantly increased and CTRP3,CTRP15 decreased in PCOS+ NIR group(P<0.05).Compared with IR group,HbA1 c,ALT,FINS,HOMA-IR,TEST,FAI were significantly increased and HDL-C,P,SHBG,CTRP3,CTRP15 decreased in PCOS+IR group(P<0.05).Compared with PCOS+NIR group,BMI,WHR,SBP,DBP,HbA1 c,TC,ALT,FINS,HOMA-IR,FAI were significantly increased and HDL-C,SHBG,CTRP3,CTRP15 decreased in PCOS+IR group(P<0.05).Correlation analysis showed that serum CTRP3 level was negatively correlated with BMI,SBP,DBP,FPG,HbA1 c,FINS,HOMA-IR,LH,TEST,FAI and positively correlated with M value(P<0.05).Serum CTRP15 level was negatively correlated with BMI,FPG,HbA1 c,TG,AST,FINS,HOMA-IR,LH,TEST,DHEAS,FAI and positively correlated with HDL-C,M value,FSH(P<0.05).M value was negatively correlated with BMI,WHR,SBP,DBP,FPG,TG,TC,LDL-C,ALT,AST,FINS,HOMAIR,TEST,FAI and positively correlated with HDL-C,P,CTRP3,CTRP15(P<0.05).Multiple stepwise regression analysis showed that FPG,FINS,HOMA-IR,DBP and LH were independent influencing factors of CTRP3(P<0.05).FPG,M value and TEST were independent influencing factors of CTRP15(P<0.05).BMI,HOMA-IR,WHR,SBP and HDL-C were independent influencing factors of M value(P< 0.05).Binary logistic regression analysis showed that CTRP15 was a protective factor for PCOS(P<0.05)and HbA1 c,FINS,TG,TEST were risk factors for PCOS(P<0.05).Conclusion: CTRP3 and CTRP15 may be involved in the development of insulin resistance and hyperandrogenemia in PCOS patients.
