| Objective: To observe the clinical efficacy of acupoint application combined with insulin in the treatment of spleen-kidney Yang deficiency type 2 diabetes mellitus,and to evaluate its safety,to highlight the role of traditional Chinese medicine in the treatment of this disease,and to provide a new method for clinical treatment of this disease.Methods: Screening patients who visited the first department of endocrinology and wards of Shijiazhuang Hospital of Traditional Chinese Medicine from October 2019 to October 2020,A total of 60 patients with spleen-kidney Yang deficiency type 2 diabetes were randomly divided into treatment group and control group by SPSS24.0 software random number generator,with 30 cases in each group.The treatment group was given insulin intensive combined with acupoint application,and the control group was given insulin intensive treatment.4 weeks as a course of treatment,after 8 weeks of 2 courses of clinical observation and treatment,Compared with two groups of weight,body mass index(BMI),fasting plasma glucose(FPG),blood sugar 2 hours after meal(2h PG),glycosylated hemoglobin(Hb A1c),fasting insulin(FINS),1 hour after a meal insulin(1h-PIns),2hours after a meal insulin(2h-PIns),fasting C peptide(FCP),1 hour after the meal C peptide(1h C-P),postprandial 2 hours C peptide(2h C-P),insulin resistance index(HOMA-IR)and average daily insulin dosage,the change of TCM symptom score.Safety was evaluated during the trial,and the occurrence of shedding and adverse events was recorded.Finally,SPSS24.0 software was used to analyze all the observed data.Results: 1.Comparison of body weight and BMI: The body weight and BMI of the two groups were increased(P<0.05),and the increase was significant in the control group(P<0.01);There was no difference between groups(P > 0.05).2.Comparison of blood glucose efficacy: intra-group comparison showed that the levels of FPG,2h PG and Hb A1 c were all decreased between the two groups,with significant differences(P<0.01);Compared with the control group,the decrease of FPG and 2h PG in the treatment group was better than that in the treatment group(P<0.05),and the Hb A1 c had no statistical significance(P>0.05).3.Comparison of efficacy of islet function: intra-group comparison showed statistically significant differences in FINS,1h-PIns,2h-PIns,FCP,1h C-P,2h C-P and HOMA-IR(P<0.05).Compared with the control group,1h-PIns and 1h C-P in the treatment group were increased,with statistical significance(P<0.05),while the other indicators did not show any difference(P>0.05).4.Comparison of the average daily dose of insulin: there was statistical significance in the comparison of the average daily dose of insulin between groups(P<0.05).5.Comparison of TCM syndrome scores: intra-group comparison showed that the total scores of TCM symptoms in both groups were significantly decreased(P<0.01);The decrease in the treatment group was better than that in the control group(P<0.05).Compared with the single symptom scores of TCM,the symptom scores of both groups were significantly decreased(P<0.01);Compared with the control group,the symptom scores of "thirsty and happy to drink","cold body and limb" and "tiredness and fatigue" in the treatment group decreased more than those in the control group(P<0.05),while there was no significant difference in the remaining symptom scores between the two groups(P>0.05).6.Comparison of the total efficacy of TCM syndrome: the total effective rate of the treatment group was 93.10%,and that of the control group was 80%,with statistical difference(P<0.05).Conclusion: Acupoint application combined with insulin intensive therapy can effectively reduce the fasting blood glucose and 2 hours postprandial blood glucose levels,improve the islet function of patients,and further reduce the amount of insulin.It can also significantly improve the clinical symptoms of patients,improve the quality of life,and good safety,can be used in clinical. |
