| Objective:Through the clinical data of the National Metabolic Management Center(MMC),analyze the related influencing factors of patients with type 2 diabetes in Zhangjiakou First Hospital,and provide a basis for further optimization of prevention and treatment strategies in clinical work in this region.Methods:Select 300 patients with type 2 diabetes managed by the MMC system in the Endocrinology Department of Zhangjiakou First Hospital from January to December 2019 as the research objects,aged 30 to 93 years,average age(48.9±5.8)years,diabetes course 0-20 years,with an average course of(10.9±8.8)years.Record the patient’s general condition,past chronic disease status and main related risk factors,including:age,gender,family history,smoking history,combined hypertension,living habits,education level;measure height and weight,calculate the body mass index(BMI).Glycated hemoglobin(Hb Alc),fasting blood sugar(FBG),2 hour postprandial blood glucose(2h PBG),and blood lipids were detected.Analyze relevant data.Results:Clinical data of type 2 diabetes patients managed by the MMC of our hospital:among the 300 patients with type 2 diabetes,most were middle-aged and elderly,male,with family history of diabetes,smoking,combined with hypertension,poor living habits,and low education.Metabolism of type 2 diabetes patients managed by the MMC of this hospital:The compliance rates of Hb A1 c,FBG,and 2h PBG are 11.0%,18.0%,and12.0%,respectively;the normal percentages of total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),and triglycerides(TG)are 27.6%,38.0%,and 24.7%,respectively;overweight people accounted for 59.0%,and obese people accounted for 9.0%.Complications of type 2 diabetes patients managed by the MMC of our hospital:There are more people with diabetic kidney disease(DKD),diabetic neuropathy(DN)and diabetic vascular disease(DVD)in the older population than in the younger population.There are more pepole with DVD in the male population than in the female population.There are more people who have diabetic retinopathy(DR),DN and DVD in smokers than non-smokers.There are more people with high blood pressure who have DR,DKD and DVD than those without high blood pressure.There are more people with poor living habit who have DR,DKD and DVD than those with good living habit.The above differences are statistically significant(P<0.05).There is no statistical difference between people with family history of diabetes and low education level who have complicated DR,DKD,DN and DVD compared with people without family history of diabetes and high education level(P>0.05).The number of people with blood glucose(Hb A1c)not meeting the standard is more complicated with DR,DKD,DN and DVD than the people with blood sugar meeting the standard;the number of people with dyslipidemia complicated with DR,DKD,DN and DVD is more than that of people with normal blood lipids;the number of people with overweight/obese people complicated with DR,DKD and DVD is more than that of people who are not overweight/obese;the differences are statistically significant(P<0.05).Analysis of influencing factors of complications in patients with type 2 diabetes managed by MMC in our hospital:The results of single factor logistic regression analysis show that the type 2 diabetes patients managed by the MMC of our hospital increase with age and BMI,and the incidence of complications increases,the smoking group is higher than the non-smoking group,the hypertension group is higher than the non-hypertension group,the bad lifestyle group is higher than the good lifestyle group,the hyperglycemia group is higher than the blood sugar standard group,and the dyslipidemia group is higher In the normal blood lipid group,the above differences are statistically significant(P<0.05);the results of multivariate logistic regression analysis show:There are statistically significant differences in the incidence of complications in patients with type 2 diabetes in Zhangjiakou No.1 Hospital in terms of age,smoking,hypertension,living habits,blood sugar,blood lipids,and BMI.(P<0.05).Conclusions:Age,smoking,high blood pressure,living habits,blood sugar,blood lipids,and BMI are all influencing factors in the occurrence of complications in patients with type 2 diabetes in Zhangjiakou First Hospital;smoking cessation should be actively taken to control blood pressure,blood sugar,blood lipids,weight,and improve Lifestyle and other related measures to reduce the occurrence and progression of diabetes complications.Analyzing the clinical data and complications of type 2 diabetes patients in Zhangjiakou First Hospital through MMC data can partially reflect the characteristics of type 2 diabetes in the region,which is of great significance to the management of type 2 diabetes in the region,and is helpful for government decisionmaking. |
