| Objectives To observe the clinical efficacy of Shengyang Qushi Tongluo Recipe combined with western medicine in treating the syndrome of yang-qi deficiency,blood stasis and phlegm-dampness in stage IV of diabetic nephropathy,to explore its mechanism of action,and to explore a new idea of combined use of drugs,to provide a new solution for the clinical treatment of this disease.Methods This study mainly focused on clinical observation,Eighty-two patients with stage IV DKD with Yang qi deficiency and blood stasis combined with phlegm dampness syndrome who met the inclusion criteria were selected(from Tangshan Hospital of Traditional Chinese Medicine January 2020 to January 2021: Department of Endocrinology or Department of Nephrology).The patients were freely separated into the matched group and unite for TCM group,with 41 patients in each group.Western medicine basic treatment in the matched group(including diet,body KG,blood pressure,glucose and lipid metabolism).The unite for TCM group: Shengyang Qushi Tongluo prescription + basic treatment.To observe the clinical effect of Shengyang Qushi Tongluo Recipe on diabetic nephropathy with IV Yang Qi deficiency and blood stasis combined with phlegm and dampness.Statistical analysis: TCM symptom score,24 hours urinary excretion rates(24h Upro),serum creatinine(Scr),estimate glomerular filtration rate(e GFR),fasting plasma glucose(FPG),2 hours postprandial blood glucose(2h PG),glycated hemoglobin A1c(Hb A1c),total cholesterol(TC),triglyceride(TG),high-density lipoprotein(HDL-L)and low densith lipoprotein(LDL-L).Results 1 In this study,82 patients with stage IV diabetic nephropathy syndrome of YANG deficiency,blood stasis and phlegm-dampness were enrolled In the control group,one patient was rejected because he failed to reexamine at the due date,40 cases actually.2Clinical total effect: control group effective in 6 cases,effective in 22 cases,ineffective in12 cases,treatment group effective in 15 cases,effective in 21 cases,ineffective in 4 cases;Clinical total effect: control group effective in 6 cases,effective in 22 cases,ineffective in12 cases,treatment group effective in 15 cases,effective in 21 cases,ineffective in 4 cases;The all cure rate of the unite for the TCM group and the matched group were 90% and70% respectively,and the unite for the TCM group had a higher cure rate(P < 0.05).3After treatment,24 hupro decreased in both groups(P < 0.05),and there was significant difference between the two groups(P < 0.05).4 TCM syndrome score: after treatment,the TCM syndrome score decreased in both groups(P < 0.05).Compared with the control group,the scores of TCM syndromes in the treatment group were significantly lower than those in the control group(P < 0.05).Compared with the control group,the treatment group had no significant difference(P > 0.05)in the scores of TCM syndromes of “Fatigue and lumbago”.5 Kidney function: The SCR decreased and EGFR increased in both groups following treatment(P < 0.05).Compared with other groups,the unite for TCM group had better effect(P < 0.05).6 Blood Sugar: FPG,2HPG and Hb A1 c in the group were lower than that before treatment(P < 0.05);The unite for the TCM group was better than the control group(P < 0.05).7 Blood Sugar: Following combined treatment,TC,TG and LDL-C were decreased and HDL-C was increased(P < 0.05).Conclusions Shengyang Qushi Tongluo decoction combined with western medicine basic treatment for diabetic nephropathy IV(yang qi deficiency and blood stasis with phlegm-dampness syndrome),compared with Western medicine basic treatment alone,it can improve TCM syndrome score,24-hour urine protein content,blood glucose,blood lipid and renal function,and has no toxic side effect,safe and effective,worth promoting.Figure 0;Table 15;Reference 108. |
