| Objectives The changes of serum nesfatin-1 level and related indexes in patients with diabetic peripheral neuropathy(DPN)were detected.The correlation between nesfatin-1and related indexes was analyzed.The relationship between nesfatin-1 and DPN as well as its influencing factors were discussed,so as to provide a possible target for the early prevention,diagnosis and treatment of DPN.Methods A total of 143 patients with type 2 diabetes mellitus(T2DM)who were hospitalized in Hebei General Hospital from March 2018 to November 2020 were selected as subjects.Among them,66 patients with diabetic peripheral neuropathy were included as the experimental group(DPN group).77 patients with simple diabetes were used as control group(DM group).General clinical data of all subjects were collected.fasting blood glucose(FBG),glycated hemoglobin(HbA1c),blood lipid,renal function and total bile acid(TBA)were measured in both groups.Enzyme linked immunosorbent assay was used to measure serum nesfatin-1 concentration.All the data were processed and analyzed using SPSS20.0 statistical software.Results 1 There were no statistically significant differences in gender,age,body mass index(BMI),systolic blood pressure(SBP)and diastolic blood pressure(DBP)between theDM group and the DPN group(P>0.05).DPN group had longer disease duration thanDM group [8.00(2.75,16.25):5.00(3.00,10.00)years],and the difference was statistically significant(P=0.048).2 There were no significant differences in total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),blood nitrogen(BUN),creatinine(Cr),uric acid(UA)and total bile acid between theDM group and the DPN group(P>0.05).The triglycerides(TG)of DPN group was higher than that ofDM group [1.75(1.14,2.88):1.26(1.01,2.03)mmol/L],and the difference was statistically significant(P=0.037).FBG in DPN group was higher than that inDM group[8.60(6.79,11.29):7.55(6.08,9.12)mmol/L],and the difference was statistically significant(P=0.021).HbA1c in DPN group was higher than that inDM group [8.90(7.58,10.80):8.00(6.55,9.50)%],and the difference was statistically significant(P=0.002).3There were significant differences in serum nesfatin-1 concentration between the two groups.Serum nesfatin-1 concentration in the DPN group was lower than that in theDM group [712.21(458.25,969.15):766.64(591.87,984.44)pg/m L],with statistical significance(P=0.028).4 Spearman correlation analysis showed that serum nesfatin-1 was negatively correlated with BMI,DBP,TC,TG,FBG and HbA1c(correlation coefficients were-0.204,-0.184,-0.176,-0.416,-0.276 and-0.297,P values were 0.014,0.027,0.036,<0.001,0.001 and <0.001,respectively).It was positively correlated with age,course of disease,BUN and Cr(correlation coefficients were 0.510,0.273,0.197,0.247,P values were <0.001,0.001,0.019,0.003,respectively),but had no correlation with sex,SBP,UA,HDLC,LDL-C and TBA(P > 0.05).5 Multiariable Logistic regression analysis showed that nesfatin-1 was the protective factor for T2DM with DPN(regression coefficient was-0.002,P = 0.031),the course of the disease and HbA1c were the risk factor for T2DM with DPN(regression coefficients were 0.101,0.233,P values were 0.001,0.030,respectively).Conclusions 1 The course of disease in the DPN group is longer than that in theDM group,and TG,FBG,and HbA1c level are higher than those in theDM group.The serum nesfatin-1 concentration in the DPN group is lower than that in theDM group.2 Serum nesfatin-1 concentration is negatively correlated with BMI,DBP,TC,TG,FBG,HbA1c in patients with type 2 diabetes,and positively correlated with age,course of disease,BUN,and Cr.3 nesfatin-1 is a protective factor,and the course of disease and HbA1c are the risk factors for T2DM with DPN.Figure 1;Table 5;Reference 202... |
