Objective:To observe the clinical efficacy of different administration methods of glucocorticoid in the treatment of acute exacerbation of chronic obstructive pulmonary disease.Methods:A collection of 88 patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)who were hospitalized in the Department of Respiratory and Critical Care Medicine,Baiyun Hospital Affiliated to Guizhou Medical University from October 2019 to December 2020 as the research objects,and88 cases that met the test criteria were collected.AECOPD patients were divided into groups and divided into inhalation group and intravenous group by random residue grouping method,with 44 cases in each group.It is based on the use of symptomatic and supportive treatments such as anti-infection,airway relaxation,cough and phlegm relieving,and oxygen inhalation,both groups were treated with glucocorticoid.inhalation group was treated with nebulized budesonide(2 mg/times,2 times/d),intravenous group was treated with methylprednisolone sodium succinate(40 mg/times,1 time/d),and both groups were treated continuously for 7 days.Record the arterial partial pressure of oxygen(Pa O2),arterial partial pressure of carbon dioxide(Pa CO2),the ratio of forced expiratory volume in the first second(FEV1/FVC),The percentage of forced expiratory volume in one second to the predicted value(FEV1%pred)and the improvement of the chronic obstructive pulmonary disease assessment test(CAT),compare the lung function,blood gas analysis index,CAT score and adverse reactions of the two groups before and after treatment The incidence rate was compared,and the clinical efficacy of different administration methods of glucocorticoid was further analyzed.Results:1.A total of 88 patients were enrolled,including 49 male patients and 39 female patients.There was no statistically significant difference between the two groups in gender,age,acute exacerbation time,and lung function classification.2.After treatment,the lung function,arterial blood gas analysis index and CAT score of the inhalation group and the venous group were improved compared with before treatment,and the difference was statistically significant(P<0.05),but the difference between the two groups was not statistically significant(P>0.05).3.After treatment,the adverse reaction rate in the inhalation group was 4.55%,and the adverse reaction rate in the intravenous group was 20.45%.Compared with the inhalation group,the incidence of adverse reactions in the intravenous group was significantly increased.(P<0.05).Conclusion:1.Glucocorticoids in the treatment of chronic obstructive pulmonary patients with acute exacerbation,no matter what method of administration is used,can achieve good clinical effects,can improve the patient’s blood gas analysis and lung function indicators,and reduce the impact of the disease on the patient.2.Compared with the intravenous group,the inhalation group has a lower chance of adverse reactions and higher safety.
