Background: Parkinson’s disease(PD)is a slow and progressive neurological degeneration disease in the elderly.It is mainly manifested as progressive motor function loss and other non-motor symptoms,such as depression,constipation,sleep disorders and so on.Moreover,the long illness duration and poor prognosis of PD has brought enormous economic and mental burden to the society and the families.Thus,It is of great significance to explore the related risk factors of PD.Rapid-eye-movement(REM)sleep behaviour disorder(RBD)is classified as a parasomnia,a disorder in which undesirable physical events and dream enactment are predominant during sleep that increases the activity of EMG.Recent studies suggest that RBD may be a particularly specific and sensitive marker for early synucleinopathies,such as PD,so the utility of the RBD scale in epidemiological investigations screening PD high-risk groups is very important and contributes a lot to the secondary prevention of the disease.Objective: To evaluate the sensitivity and specificity of the Chinese version of RBD1 Q questionnaire(RBD1Q-C),and to observe whether the RBD1Q-C questionnaire can be used for clinical screening.And to explore the possible association between RBD and PD,find out other risk factors of PD,and provide scientific basis for the study of etiology and prevention of PD.Methods: 1)Patients with sleep problems were enrolled in neurology,respiratory clinics and wards,and the patients were interviewed with RBD1Q-C sacle,followed by polysomnography(PSG)test to diagnosis if the patients had RBD.2)An epidemiological survey was conducted in the Malu community in Shanghai,and the RBD(p RBD)patients were screened by the RBD1Q-C scale to calculate the prevalence.3)Based on the United Kingdom PD Society Brain Bank Clinical Diagnostic Criteria,two-step screening method was used to screen and diagnose PD patients in Malu community.4)Though retrospective analysis of the basic demographic data and lifestyle conditions of the patients and controls,we screened and then picked out potential risk factors of PD.In addition,we analyzed whether there is a correlation between RBD and PD.Results: 1)The test result for RBD1Q-C scale was sensitivity 100%,specificity 55.6%.The p RBD prevalence of Shanghai rural population was4.9%,and p RBD was associated with odds of Parkinson’s disease.2)After adjusting potential confounders,we found that the history of coronary artery disease(CAD)was associated with a high risk of PD(adjusted OR =9.62,p <0.001),and stroke was also associated with a high risk of PD(adjusted OR = 8.09,p <0.001).Conclusion: The RBD1Q-C scale has high sensitivity and can be used in the epidemiological screening of large samples.The screening of p RBD is of great value in the early diagnosis of PD.CAD and stroke are associated with the risk of PD.
