Collection And Data Mining Of Headache Medical Records Of Famous Modern Doctors In Lingnan Area

ObjectiveBy sorting out the experience of Modern Lingnan famous traditional Chinese medicine in the diagnosis and treatment of headache,this paper analyzes the characteristics,diagnosis and drug use of headache in Lingnan area,discusses the related etiology,pathogenesis and syndrome types,and grasps the law of Modern Lingnan headache treatment as a whole,so as to provide clinical basis for the diagnosis and treatment of headache in Lingnan area in the future.Methods1.Literature collection.To collect the Modern Lingnan famous traditional Chinese medicine cases,the inclusion criteria are as follows:the medical works and medical cases of the famous old traditional Chinese Medicine collected in this project are all studied by the "Guangdong famous old traditional Chinese medicine" titles announced by the people’s Government of Guangdong Province since the 1960s,as well as the academic experience inheritance work guidance teachers of the old traditional Chinese medicine experts determined by the two ministries and one bureau of the State Council.Through the collection in the library and Literature Library,and the search and download of the periodical literature database,the existing medical cases of the treatment of headache by the famous old traditional Chinese medicine in Modern Lingnan were collected and sorted out.2.Document arrangement.All the documents collected are sorted and distinguished according to the category,and the relevant data are input into the table through Excel software.SPSS 21.0 and R(3.6.1)are used to analyze the Apriori association rules.Frequency analysis,factor analysis,system cluster analysis and Apriori association rules are used to analyze the collected data.The rules of headache treatment are systematically studied,and the connotation of Lingnan famous doctors’ clinical treatment ideas and medication rules are analyzed by combining professional knowledge.Results1.Collection and arrangement of medical records:a total of 62 famous old Chinese doctors in Lingnan,28 volumes of headache related works and 98 literatures were collected in this project.A total of 164 medical records and 282 effective prescriptions were collected.2.Data mining results of headache medical records:there are 86 kinds of symptoms in headache medical records,and the total frequency of symptoms is 1256.There are 27 kinds of symptoms with frequency ≥ 5,the total frequency is 898,the cumulative frequency is 71.5%.The main clinical symptoms are headache,dizziness,heavy head,tinnitus,insomnia,nausea and vomiting,fidgety,poor appetite,neck discomfort(swelling,pain,tightening),etc.There are 28 kinds of tongue images and the total frequency is 98.Among them,the frequency of tongue thinning was the highest,accounting for 17.61%,followed by moss white,red,dark,greasy,yellow,thin,fat and thick(frequency≥10).There are 11 kinds of pulse in medical cases,and the total frequency is 188.Among them,the highest is pulse string,accounting for 36.2%,followed by pulse fine,pulse number,pulse sliding,pulse sinking,pulse slow,pulse floating,pulse weak(frequency>5).There are 22 types of syndromes in medical cases,and the total frequency is 349.In this syndrome,the frequency of hyperactivity of liver Yang is the highest,accounting for 23.78%,followed by wind cold headache,blood stasis headache,phlegm dampness and internal obstruction,wind evil disturbance,liver fire inflammation,wind heat headache,qi stagnation and blood stasis,phlegm and blood stasis,Shaoyang headache(frequency ≥10).119 drugs were shared and 3191 times were used.In fact,the frequency of Ligusticum chuanxiong is the highest,122;the second is Paeonia alba,99;the third is Poria cocos,91.Factor analysis of 38 drugs with the first frequency showed that 10 drug combinations could be obtained.By cluster analysis,7 kinds of prescription combinations were obtained,which have the functions of calming the liver and suppressing yang,eliminating wind and pain,tonifying the spleen and promoting Qing,promoting blood circulation and removing blood stasis,and dispersing the liver and relieving depression.Through the analysis of association rules,it is found that the corresponding rules of prescriptions and syndromes in headache medical cases,such as Gastrodia elata,uncaria,Paeonia alba,Scutellaria,cassia,etc.are commonly used in the syndrome of hyperactivity of liver Yang,which implies that Gastrodia elata and Uncaria drink are used to calm the liver and latent Yang;Ligusticum chuanxiong,Angelica dahurica,Fangfeng,Qiang Huo,Vitex,etc.are commonly used in the syndrome of wind cold headache,which is similar toqingshangtong decoction,which is commonly used in the prescription of dispelling wind,dispersing cold and relieving pain;blood stasis headache Commonly used Taoren,Honghua,chuanxiong,Dilong,red peony and other drugs,for Xuefu Zhuyu Decoction;Yiqi Shengyang with Pueraria,Beiqi,Baizhu,similar to Huangqi Gegen Decoction;phlegm dampness internal resistance commonly used pinellia Baizhu,Zhuru,similar to Wendan Decoction,reflecting the basic treatment of phlegm dampness group;Jianpi Buqi commonly used Poria,dangshen,Huangqi,Baizhu,similar to Sijunzi Decoction;Shugan Jieyu often It is composed of white peony,bupleurum,Fructus aurantii and Alisma orientalis,similar to Bupleurum Shugan powder.These commonly used drugs and prescriptions reflect the basic thinking characteristics of Lingnan doctors,and also reflect the common basic syndromes of headache in Lingnan area.Conclusion1.In Lingnan area,the headache is mainly caused by liver,the pathogenic factors are mainly solid,and the syndrome type is mainly hyperactivity of liver Yang,which is followed by wind cold headache,blood stasis headache,phlegm dampness and internal obstruction,wind evil shangharass,liver fire Shangyan,wind heat headache,qi stagnation and blood stasis,Phlegm Blood Stasis and phlegm heat.2.The treatment of headache mainly includes eliminating wind and pain,calming the liver and Yang,nourishing the spleen and Yang,dispersing the liver and Qi,promoting blood circulation and stasis,eliminating wind and phlegm.2.The treatment of headache mainly includes calming liver Yang,eliminating wind and pain,promoting blood circulation and removing stasis,benefiting qi and Promoting Yang,resolving phlegm and turbidity,strengthening spleen and nourishing qi,and relieving liver depression.3.The results of data mining indicate that the common components of the disease are:Gastrodia and Uncaria Decoction,qingshanghuantong decoction,Xuefu Zhuyu Decoction,Huangqi Gegen decoction,Sijunzi Decoction,Wendan Decoction,Chaihu Shugan powder and Xiaoyao Pill.
