| Objcetive:With clinical diagnosis and treatment of distal tibiofibula joint ligament injury after spin-spin(SER)Ⅳ ankle fractures are common,often in processing because of the emphasis on the bony structure of fixed but ignore the processing of ligament structure,thus will bring some late complications,based on the physique and under the guidance of the theory of distal tibiofibula joint structure using different approach to the comparative analysis to select the most suitable processing method.Methods:Selection in September 2017 to July 2019 in shandong university of Chinese medicine affiliated hospital wards in the hospital surgical treatment of SER IV ankle fracture patient,and determined by intraoperative probe tibiofibula,a total of 100 patients with distal joint of structural damage,after fixed bony structure depending on the distal tibiofibula joint structure processing mode is divided into two groups,a stable group of 50 cases,unstable group of 50 cases,including 25 cases of unstable group is divided into screw,repair group 25 cases.The distance between the distal end of tibiofibula and the overlapping distance of the distal end of tibiofibula on the anklet positive X-ray were measured before surgery,after surgery and half a year after surgery,respectively.The ankle AOFAS ankle score of the three groups in the six months after surgery was compared,and the data obtained were processed and analyzed using SPSS24.0 software.Results:The osseous structures of each group were healed successfully,including 2 cases of blade infection in the stable group,3 cases of screw fracture in the screw group,3 cases of re-widening of distal tibiofibular joint in the screw group,and no re-widening of distal tibiofibular joint in the repair group.From the perspective of ankle function half a year after surgery,the functional score of the repair group was higher than that of the screw group,and the overall score of the unstable group was higher than that of the stable group.The distance between the distal ends of the tibiofibula and the overlapping distance of the distal ends of the tibiofibula in each group were P<0.05 in the imaging values of the six months after surgery and six months after surgery,and the difference was statistically significant.Conclusion:For joint structure damage associated with distal tibiofibula SER ankle fractures by repair with Ⅳ tibiofibula distal ligaments before and attached bony structure can have the effect of reinforcement tibiofibula distal joint.Compared with other treatment methods,it is more simple and convenient and conforms to the characteristics of biomechanics and anatomy.It does not need secondary surgery and is worthy of clinical promotion and application.Due to insufficient clinical observation time,the number of follow-up cases and the relative lack of data in this study,the long-term clinical treatment results still need to be further observed and discussed. |
