Objective: The difference between ultrasound guided needle-knife therapy and general acupuncture therapy in patients with knee osteoarthritis and synovitis was observed.To explore the effectiveness and superiority of ultrasound guided needle-knife therapy,so as to provide clinical ideas for the treatment of knee osteoarthritis and synovitis in the future.Methods: Selecting during January 2018 to December 2019,60 patients who met the inclusion criteria in the rehabilitation department of Shijiazhuang traditional Chinese Medicine Hospital were selected as subjects.They were divided into 30 cases of ultrasound needle-knife group(treatment group)and 30 cases of general acupuncture group(control group)according to the method of random digital table.The ultrasonic needleknife group was treated with small needle-knife release under ultrasonic guidance,once a week for a total of 3 weeks;the general acupuncture group received ordinary acupuncture treatment(acupoint selection: Xuehai,Ququan,Yinlingquan,Yanglingquan,Dubi,Liangqiu,Zusanli,Ashixue),once a day for 6 times a week for a total of 3 weeks.Observation indicators:1 the VAS score and WOMAC score before and after treatment in the two groups;2 the value of synovial thickness and joint effusion before and after treatment in the two groups;3 the improvement of clinical symptoms in different treatment methods in the two groups.Both shedding and adverse events were also recorded.Subsequently,SPSS23.0 statistical software was used to analyze the data and evaluate the efficacy.Results: 1.Joint pain: the VAS score before and after treatment was used to investigate the remission of knee pain in the two groups,whichwere significantly different from that before and after treatment(P<0.01).After treatment,the scores of both groups were decreased,and the difference between the two groups was significant(P < 0.01).2.Joint function: by comparing the indexes of WOMAC osteoarthritis before and after treatment,there were significant differences between the two groups in joint pain,stiffness,daily activity function and WOMAC total score(P<0.01).After treatment,the scores of the two groups was decreased,and there was a statistical difference between the two groups(P < 0.01).3.Ultrasono-graphic results: The ultrasonic evaluation of synovial thickness and joint effusion depth between the two groups before and after treatment showed statistically significant difference(P < 0.01).After treatment,all values of the two groups decreased,and there was a statistical difference between the two groups(P < 0.05).There was a positive correlation between synovial membrane,effusion and VAS score and WOMAC score in both groups.4.Total efficiency: the total effective rate of ultrasonic needle-knife group was 92.59%,which was general acupuncture group was75.00%,and there was statistical difference between the two groups(P<0.05).Conclusion: Ultrasound guided needle-knife therapy and general acupuncture therapy in the treatment of knee osteoarthritis and synovitis have clinical efficacy in joint pain,joint stiffness,daily motion,synovial thickness and joint effusion depth,and ultrasound guided needle-knife therapy has better clinical efficacy,which can significantly relieve pain and improve joint motion function in a short term.Ultrasound technology can play an advantage in the process of acupotomology treatment,reflecting the objective changes of synovial thinning and fluid accumulation reduction,can be used as a quantitative evaluation technology,worthy of clinical promotion.
