Background:In the process of allogeneic kidney transplantation,the transplanted kidney will definitely be affected by ischemia-reperfusion injury.The ischemia-reperfusion injury of the transplanted kidney is the main reason for the acute rejection of the transplanted kidney and the delayed recovery of graft function,and it also affects the transplantation.One of the important reasons for the long-term survival of the kidney,how to effectively reduce the ischemia-reperfusion injury of the transplanted kidney has always been the focus of clinical work.In recent years,the repairing effect of bone marrow mesenchymal stem cells has become a hot spot for clinical and basic research.Because of its wide source,easy industrial preparation,and low immunogenicity,it has been widely used in tissue engineering,biotherapy and other fields of research However,the therapeutic effect of bone marrow mesenchymal stem cells is closely related to the number of homing to damaged sites and their ability to survive in the local microenvironment.Therefore,using some pretreatment methods to improve the homing ability and survival ability of bone marrow mesenchymal stem cells is a new way to improve their therapeutic effect.Studies have shown that bone marrow mesenchymal stem cells express erythropoietin receptors.Pretreatment of bone marrow mesenchymal stem cells with erythropoietin can enhance their homing ability and promote damage repair ability,but the specific mechanism of action needs to be further Research and discussion.Purpose:Using cell experiments and animal experiments to explore the effect of erythropoietin on the proliferation and differentiation of bone marrow mesenchymal stem cells,and further explore the mechanism of bone marrow mesenchymal stem cells after renal preconditioning pretreatment of ischemia-reperfusion injury.Method:1.Cell experiment;co-cultivation of bone marrow mesenchymal stem cells and erythropoietin(500 IU/mL)in the homogenate supernatant of ischemia-reperfusion injury kidney,grouping and labeling,and then performing bone marrow Experiments such as osteogenesis,adipogenic differentiation and apoptosis observation of mesenchymal stem cells explore the effects of erythropoietin on the proliferation and differentiation of bone marrow mesenchymal stem cells.2.Animal experiments:Experimental animals were divided into model group(clamping bilateral renal pedicles for 45 minutes,causing ischemia-reperfusion injury),BMSCs group(model rats injected with bone marrow mesenchymal stem cells via tail vein),EPO group(model mice via tail Intravenous injection of erythropoietin),EPO+BMSCs group(bone marrow mesenchymal stem cells pretreated with erythropoietin through the tail vein).The kidney tissues of rats were obtained for pathological examination on the first day and the fifth day after the operation,and the blood of the inferior vena cava was collected for renal function test to explore the therapeutic effect of EPO+BMSCs on renal ischemia-reperfusion injury.Result:1.EPO does not change the surface markers of BMSCs.After osteogenic induction of BMSCs group and EPO-BMSCs group,red mineralized nodules can be seen;BMSCs group and EPO-BMSCs group after induction of adipogenesis There are more obvious round small lipid droplets,which are stained with oil red O,and red lipid droplets can be seen;2.In the microenvironment of renal ischemia-reperfusion injury,EPO can significantly reduce the apoptosis rate of BMSCs;3.After 1 day and 5 days of treatment with EPO,BMSCs and EPO-BMSCs,the EPO-BMSCs treatment group showed a lighter inflammatory response,and the score was significantly different from the model group;4.In the EPO-BMSCs treatment group,the renal function of rats was significantly improved.In conclusion:1.Erythropoietin pretreatment can enhance the anti-apoptotic ability of bone marrow mesenchymal stem cells in the damaged microenvironment;2.Pretreatment of bone marrow mesenchymal stem cells with erythropoietin can significantly improve renal ischemia-reperfusion injury.
