Discussion On The Medication Rule Of Professor Gong Hongwei In Treating Colorectal Cancer Based On Data Mining Technology

ObjectiveThis study uses the data mining technology to explore Professor Gong Hongwei’s medication rules in the treatment of colorectal cancer,discover the core drug pairs and prescriptions,and summarize the academic thoughts of Professor Gong Hongwei in the diagnosis and treatment of colorectal cancer based on clinical experience,with a view to providing some reference for the treatment of colorectal cancer with Chinese medicine.MethodThis study collected a total of 80 patients with colorectal cancer in the outpatient and inpatient departments of Professor Gong Hongwei of Hubei Provincial Hospital of Traditional Chinese Medicine from January 2019 to January 2021,with a total of 157 visits.First,establish a data database,and then use Microsoft Excel,SPSS 22.0 and SPSS Modeler 18.0 statistical software to conduct statistics through frequency analysis,cluster analysis and association rules.Based on statistical results,combined with relevant clinical practice,this study discussed the rules of prescription and medication and academic thoughts of Professor Gong Hongwei’s differentiation and treatment of colorectal cancer.Result(1)According to the gender statistics of patients,the male to female ratio is about 1.42:1,and the age statistics show that 65-69 years old are the most prevalent age group,accounting for 32.5%of the total number.(2)According to the eighth edition of AJCC colorectal cancer TNM staging,there are 67 cases of stage IV,6 cases of stage III,6 cases of stage II,and 1 case of stage I.According to the statistics of the site of the disease,36 cases occur in the rectum area,accounting for 45%;16 cases occur in the sigmoid colon area,accounting for 20%.In the pathological classification,there are79 cases of adenocarcinoma and 1 case of squamous cell carcinoma.Western medicine treatments show that there are 54 patients with history of surgery and 26 patients without it,accounting for 67.5%and 32.5% respectively;47 patients with history of chemotherapy and 33 patients without it,accounting for 58.75% and 41.25%respectively;10 patients with history of radiotherapy and 70 patients without it,accounting for 12.5% and 87.5% respectively;12 patients with history of targeted therapy and 68 patients without it,accounting for 15% and 75% respectively;2 patients with history of immunotherapy and 78 patients without it,accounting for 2.5%and 97.5% respectively.(3)According to the statistics of TCM symptoms of patients with colorectal cancer,the main symptoms of this disease are fatigue(73 times),anorexia(73 times),abdominal pain(64 times),abdominal distension(56 times),poor sleep(43 times),chest tightness(35 times),palpitation(25 times),frequent bowel movements(25 times),anal bulging(21 times),and dry mouth(21times).(4)According to the statistics of tongue and pulse conditions,slight red tongue,thin yellow moss,dark red tongue,red tongue,white moss,thin white moss,yellow moss,pale tongue,and yellow greasy moss are more frequent in the case of the tongue;wiry pulse,slippery pulse,heavy pulse,and slippery and rapid pulse occur more frequently.(5)According to the syndrome statistics,they are successively ranked as follows: stasis and toxin internal stagnation syndrome(51 times),damp-heat accumulation syndrome(44 times),spleen-stomach disharmony syndrome(30 times),qi and blood deficiency syndrome(17 times),spleen and kidney yang deficiency syndrome(8 times),and liver and kidney yin deficiency syndrome(7 times).(6)According to the statistics,a total of 276 kinds of drugs are involved in the prescription medication,which are used for 3260 times in total.A total of 50 Chinese medicines are used with the frequency ≥20 times,among which the top 10 drugs are Baizhu(118times),Fuling(100 times),Chenpi(78 times),Gancao(78 times),Baishao(77 times),Huangqi(71 times),Danggui(68 times),Banzhilian(66 times),Baihuasheshecao(65 times)and Muxiang(65times).(7)The drugs with the use frequency ≥20 times are further divided into 14 categories according to the efficacy of Chinese medicinals,and the three most frequently-used drugs are the heat-clearing drugs,the deficiency-tonifying drugs and the qi-regulating drugs.The heat-clearing and detoxicating herbs are the main ones among heat-clearing herbs,while qi-reinforcing herbs are the main ones among deficiency-reinforcing herbs.(8)In the statistics of drug-induced taste and meridian tropism,the frequency of use of the four qi in the top three are warm(35.74%),cold(30.55%)and mild(25.71%).The top three using frequencies of the five flavors are sweet(30.23%),bitter(28.4%)and pungent(22.38%).The five most frequently-used meridian tropisms of drugs are in the spleen meridian(20.46%),liver meridian(16.86%),stomach meridian(14.98%),lung meridian(13.83%),and kidney meridian(9.67%).(9)Association rule analysis is performed on high-frequency drugs.The top 5 confidence levels of percentile ranking are: Baizhu–Wuweizi(97.44%),Baizhu–Taizishen–Fuling(97.14%),Baizhu–Huangqi–Gancao–Fuling(97.06%),Banzhilian–Baihuasheshecao– Chenpi(96.88%)and Baizhu – Fangfeng(94.60%).The top 5supporting degrees of percentile ranking are: Baizhu – Fuling(63.06%),Baihuasheshecao – Banzhilian(42.04%),Banzhilian –Baihuasheshecao(41.40%),Baizhu–Chenpi–Fuling(35.67%)and Baizhu–Gancao–Fuling(34.40%).(10)Clustering mining of 157 clinical prescriptions,this study summed up Professor Gong Hongwei’s prescription and medication rules in the treatment of colorectal cancer.Based on Professor Gong’s clinical experience for many years,it was summarized that the main syndrome differentiation types of colorectal cancer were stasis and toxin internal stagnation syndrome,damp-heat accumulation syndrome and spleen-stomach disharmony syndrome.During the treatment,emphasis was placed on strengthening the body resistance and consolidating the constitution,anti-cancer and detoxification,and attention was paid to protecting the spleen and stomach function.Sijunzi Decoction was often modified to invigorate the spleen and replenish qi,and Banzhilian-Baihuasheshecao were often used for anti-cancer and detoxification.Attach importance to dredging fu-organs,and take dredging as application.When treating blood syndrome of colorectal cancer,hemostatic drugs should be flexibly selected to stop bleeding without leaving any blood stasis.In the process of diagnosis and treatment,cold and heat should also be distinguished,and Yiyi Fuzi Baijiang Powder and Dahuang Mudan Decotion were used appropriately to modify the syndromes according to the syndromes.
