Clinical Observation Of Exercise Therapy Combined With Traditional Chinese Medicine In The Treatment Of Early Necrosis Of The Femoral Head

OBJECTIVE: Through clinical observation and study,to observe the changes of pain and joint function of patients with early osteonecrosis of the femoral head before and after(ONFH)treated with exercise therapy combined with traditional Chinese medicine,and then analyze the treatment results and draw a conclusion.METHODS: A total of 30 patients with early osteonecrosis of the femoral head were diagnosed and treated in the Department of Orthopaedics and Traumatology of Traditional Chinese Medicine Hospital of Wuhan city from January 2019 to October 2020(including19 males and 11 females;42 hips: 12 cases of bilateral necrosis,11 cases of right necrosis and 7 cases of left necrosis).Fifteen patients(9 males and 6 females)in the treatment group were treated with exercise therapy combined with traditional Chinese medicine.The age of the patients was 42.52 ±4.32 years old,and the course of disease was 10.32 ±3.15 months.Fifteen patients in the control group(10 males and 5 females)were treated with traditional Chinese medicine,the age was 41.45 ±5.54 years old,and the course of disease was 9.89 ±4.36 months.During the treatment,two groups of patients were required to walk on crutches and rest in bed in order to reduce the weight of the femoral head.Stop any other drugs or treatments related to this treatment as required during the treatment.After observation,revisit and follow-up for 3 months,the VAS pain score,Harris score,joint effusion and bone marrow edema score of the two groups were recorded before treatment,1month after treatment and 3 months after treatment,and then the statistical methods were used to analyze the data.RESULTS: All the patients in the two groups completed the clinical observation according to the requirements,Recorded and preserved the complete evaluation index data.After treatment,observation and analysis,it was found that in the treatment group,there was significant difference in VAS score before treatment and 1 month after treatment(P < 0.05),and there was significant difference in VAS score between 1 month after treatment and 3 months after treatment(P < 0.05).The two results showed that the treatment group could not only quickly relieve joint pain but also keep patients in a state of low pain.In the control group,the VAS scores before treatment and 1 month after treatment were compared(P >0.05),and the difference was not statistically significant,indicating that the pain was not improved significantly.After 1month of treatment,the pain began to decrease significantly.After1 month and 3 months of treatment,there was a significant difference in the VAS score between the two groups(P < 0.05),indicating that the treatment group was significantly better than the control group in the treatment of early osteonecrosis of the femoral head.In the comparison of Harris score,there was a significant difference between the two groups after 1 month and 3months of treatment(P < 0.05),which means that the treatment group was significantly better than the control group in improving the function of hip joint.There was a significant difference in the scores of joint effusion and bone marrow edema between the two groups after 1 month and 3 months of treatment(P < 0.05),indicating that the improvement of the treatment group was better than that of the control group.Conclusion: Exercise therapy combined with traditional Chinese medicine is effective in the treatment of early osteonecrosis of the femoral head,which can quickly relieve hip pain in a short time,and can significantly improve the range of motion of hip joint,joint effusion and bone marrow edema.So that patients can engage in basic life and improve the quality of life,indicating that the treatment program can be popularized and applied in clinic.
