| Objective: To investigate the efficacy and related influencing factors of port wine stains(PWS)patients treated with Hemporfin Photodynamic(HMME-PDT)once,and three non-invasive examination methods to use HMME-PDT in PWS patients The application value in treatment,that is,whether dermoscopy can be used as a way to predict photodynamic efficacy;high-frequency ultrasound measurement of skin lesion density,CK detector to measure erythema index(EI value)are used to quantitatively evaluate the efficacy of HMME-PDT in the treatment of PWS patients.Methods: Select 28 patients who were clearly diagnosed as PWS in our department and received treatment.After HMME-PDT treatment for one time,combined VISIA and photos to observe the efficacy and related influencing factors;and use dermoscopy to observe the blood vessels in the same skin lesion before and after treatment To explore the feasibility of dermoscopy for predicting the efficacy of HMME-PDT in the treatment of PWS.At the same time,14 patients with PWS were used to measure the density of the same skin lesion before and after the treatment(face IV area)and the different points of the normal part on the opposite side,and the CK detector was used to measure the difference of the same skin lesion before and after the treatment.Point EI value(measure 3 points),take the average value,and use EI difference(ΔEI)to quantify the difference.By evaluating the density difference of the skin lesions before and after treatment and the ΔEI value of the skin lesions,discuss the high-frequency ultrasound and CK detector quantitatively evaluates the feasibility of HMME-PDT in the treatment of PWS.At the same time,follow-up monitoring of patients’ adverse reactions during and after treatment.RESULTS: 1.The overall effective rate of 28 PWS patients after one HMME-PDT treatment: 89.3%.There was no statistical difference in the efficacy in terms of gender and the previous history of laser treatment(P>0.0 5);the efficacy was statistically different in age,skin lesion distribution,and type of skin lesions(P <0.0 5).2.Observed by dermoscopy before and after treatment,it was found that after HMME-PDT treatment: the curative effect of superficial vascular group,deep vascular group and mixed(superficial + deep)vascular group was statistically different(P < 0.0 5),and the effect is superficial>mixed>deep vascular group.3.Under high-frequency ultrasound,the density of skin lesions increased after treatment compared with before treatment,and there were differences in curative effect between groups.(P <0.0 5).4.CK skin tester compares the EI value before and after treatment,and found that after treatment,the EI value is lower than before treatment,and there is a difference in curative effect between groups(P <0.0 5).Conclusion: This study found that: for PWS patients,HMME-PDT treatment is effective.At the same time,dermoscopy can be used as a way to predict the curative effect of HMME-PDT in the treatment of PWS by evaluating the depth of blood vessels;high-frequency ultrasound measurement of skin lesion density and CK detector measurement of erythema index(EI value)can be used to quantitatively evaluate HMME-PDT Efficacy of treating PWS patients.The above non-invasive examination methods can objectively evaluate the clinical efficacy of HMME-PDT and provide a basis for the formulation of individualized photodynamic therapy. |
