Clinical Study On The External Treatment Of Ulcerative Lymph Node Tuberculosis With Discarding Toxin And Promoting Tissue Regeneration Method

Objective:1.Clarify the clinical efficacy and safety of discarding toxin and promoting tissue regeneration method for ulcerative lymph node tuberculosis,and further confirm the therapeutic advantages of compound Wufengcao liquid;2.Establish TCM external treatment rules and drugs for the treatment of ulcerative lymph node tuberculosis that can be promoted and applied,and establish a clinical efficacy evaluation system that reflects the characteristics and advantages of TCM.Methods:A total of 126 patients who met the criteria were divided into three groups according to the sequential and randomized control method:treatment group(Compound Wufengcao liquid for external use),kangfuxin group(Kangfuxin liquid for external treatment)and isoniazid group(isoniazid injection for external use),with 42 patients in each group.All patients were treated with basic anti-tuberculosis combined with local dressing change.The duration of the study was 28 days.The major efficacy indicators,such as wound healing rate,secondary efficacy indicators,such as decay time,TCM syndrome score,and blood routine,abnormal liver and kidney function were monitored every week.Blood samples were collected weekly before and during treatment to observe laboratory immune-related indexes.Results:1.The total effective rate of the treatment group was 92.86%,the total effective rate of the kangfuxin group was 85.71%,and the total effective rate of the isoniazid group was 76.16%,indicating that the total effective rate of the treatment group was higher than the other two groups,and the difference was statistically significant(P<0.05 or P<0.01).2.Compared with the kangfuxin group and the isoniazid group,the treatment group could effectively improve the wound healing rate,shorten the decolonization time,and reduce the TCM syndrome score,and the difference was statistically significant(P<0.01).3.Compared with kangfuxin group and isoniazid group,the protein and mRNA expression levels of iNOS,TNF-α and mRNA in plasma in the treatment group were decreased effectively(P<0.05 or P<0.01),and the protein and mRNA expression levels of Arg-1 and TGF-β in the treatment group were increased((P<0.05 or P<0.01).Conclusion:1.The compound Wufengcao liquid has obvious curative effect on the treatment of ulcerated lymph node tuberculosis,and can improve the wound healing rate,shorten the time of decay,and improve the scores of traditional Chinese medicine syndromes without obvious adverse reactions.The pain intensity during dressing change is lower,and the clinical application is safe and reliable.2.The compound Wufengcao liquid has certain advantages in regulating the polarization of macrophages,which can effectively inhibit the polarization of M1 macrophages and promote the polarization of M2 macrophages,and accelerate the healing of ulcer wounds.3.The treatment of ulcerative lymph node tuberculosis with discarding toxin and promoting tissue regeneration method can reflect the characteristics and advantages of traditional Chinese medicine and can be popularized and applied.
