Objective: To investigate whether there is a difference in the formation of venous thromboembolism(VTE)in the lower extremities between open surgery(OH)and laparoscopic surgery(MH)in patients with endometrial cancer(Endometrial Cancer,EC),and analyze the intrauterine the high incidence of thrombosis in the veins of the lower extremities after surgery in people with membrane cancer,so as to find the best time to check for venous thrombosis after surgery.Secondary purpose: To study the effects of two types of surgical methods on the coagulation function of patients with endometrial cancer,and the effects of intraoperative urine output,blood loss,and intravenous fluid infusion on the formation of lower extremity venous thrombosis in patients with endometrial cancer.Methods: This study collected data on patients with endometrial cancer who underwent laparotomy or minimally invasive surgery in the gynecological ward of the Affiliated Hospital of Jining Medical College.General conditions,preoperative blood routine,coagulation routine,preoperative and postoperative anticoagulant application,preoperative and postoperative lower extremity venous thrombosis,and the time for the first discovery of lower extremity venous thrombosis after surgery;analysis of the postoperative lower extremity vein occurrence and The influence of different surgical procedures on the coagulation function of patients;analysis of the possible time of lower extremity venous thrombosis in patients with endometrial cancer,and analysis of the influence of intraoperative urine volume,blood loss,and fluid infusion on lower extremity venous thrombosis.Results: 1)A total of 166 cases of minimally invasive surgery,17 cases of VTE occurred after the operation,the incidence rate was 10.24%,of which 0 cases of PTE;A total of 83 cases of open surgery,a total of 12 cases of VTE occurred after the operation,the incidence rate was 10.24%14.46%,1 case of PTE;There was no significant difference in the incidence of VTE between the MH surgery group and the OH surgery group(P>0.05);2)In 8 patients with endometrial cancer,blood clots in the lower limbs were detected by color Doppler ultrasound within 6 hours after the operation.Among them,1 case was diagnosed by color Doppler ultrasound within 2 hours after the operation,and 1 case was diagnosed by color Doppler ultrasound within 3 hours after the operation.Thrombosis was formed;13 cases were diagnosed by color Doppler ultrasound within 6-24 hours after operation;7 cases were found to be thrombosis within 1-3 days after operation;1 case was found to be thrombosis within 3-7 days after operation.3)The coagulation function of the two surgical methods before and after the operation was compared and analyzed.Evaluation indicators and CRP(C-reactive protein),in the MH group,the coagulation function evaluation indicators before and after surgery were compared and analyzed:D-D and FIB increased after surgery compared with before surgery(P<0.05),platelets decreased significantly after surgery compared with before surgery(P<0.05);Comparative analysis of coagulation function evaluation indicators before and after surgery in the OH group: D-D manifested as an increase after surgery compared to before surgery(P<0.05),and platelet manifestation was significantly reduced after surgery compared with before surgery(P<0.05).In the MH group and the OH group,the APTT and PT before and after the operation were compared and analyzed,and the difference was not significant(P>0.05);4)After adjusting for confounding factors such as age and BMI of the two groups of patients,multiple regression model analysis,It was found that there were significant differences between the thrombosis group and the nonthrombosis group in the surgical treatment of endometrial cancer patients in terms of operating time,intraoperative urine output,intraoperative blood loss,and whether intraoperative blood transfusion was performed(P<0.05).Conclusion: 1.Among the population undergoing endometrial cancer surgery,there is no significant difference between open surgery(OH)and laparoscopic surgery(MH)in the formation of venous thromboembolism(VTE)of the lower extremities(P>0.05);2.Uterus in patients with endometrial cancer,the common occurrence time of lower extremity venous thrombosis is 6-24 hours after operation.
