| Objective:To study the pathogenic factors, pathological types, clinical distribution characteristics and correlation of TCM syndromes of colorectal polyps,Provides some references for the prevention and treatment of colorectal polyps from the perspective of TCM.Methods:Collect data on patients who were hospitalized in the Gastroenterology Department of Jiangsu Provincial Hospital of Integration Chinese and Western Medicine from October 2019 to December 2020,Screen 178 cases that meet the inclusion criteria for investigation and research,Collect the general information of the case, the information of the fourth diagnosis, colonoscopy performance, pathological classification and laboratory examination results, etc,Fill in the clinical observation form, summarize the statistical data and establish a database, apply SPSS 22.0 software for statistical analysis.Results:1.General situation:The male to female ratio is 2.42:1,and males have more incidences than females. The age range is 26-83 years old, with an average age of 58.99±11.23 years old.The age of onset is more common in the elderly.A total of 86 cases had a history of smoking,of which 76 had a history of smoking for more than 20 years;a total of 48 cases had a history of drinking,of which 41 had a history of drinking for more than 20 years. The distribution of BMI values is common in normal weight and overweight, followed by obesity and underweight. The distribution of daily exercise volume is mainly 2,000-5,000 steps and 50-10,000 steps, followed by daily exercise volume of less than 2,000 steps and greater than 10,000 steps.A total of 99 cases of abnormal blood lipid levels were counted, among which the low level of HDL-C was the main one.2.Incidence frequency, colonoscopy appearance, pathological type:The number of first-onset cases is more than recurrent cases;the number of polyps under endoscopy is more frequent than single-onset cases;polyps size<0.5cm are the most common, followed by 0.5cm-1.0cm,1.0cm-2.0cm,and>2.0cm;polyp morphology Yamada type I is the most common, followed by Yamada type Ⅲ,Ⅱ,and Ⅳ respectively.Sigmoid colon and rectal polyps are the most common disease sites, followed by transverse colon, descending colon, ascending colon, and ileocecal area.In the pathological classification, tubular adenomas accounted for the highest proportion, followed by hyperplastic polyps, inflammatory polyps, tubular villous adenomas, and villous adenomas.3.Correlation analysis of smoking, drinking, blood lipid indexes and pathological typing:smoking, drinking, and HDL-C reduction are related to pathological typing, which will increase the risk of adenomatous polyps.4.Correlation analysis between age and incidence:There is a correlation between age and incidence, and elderly patients are more likely to relapse colorectal polyps.5.Distribution of TCM syndrome types:Damp-heat and stasis syndrome, spleen deficiency and dampness stagnation syndrome are the most common, followed by spleen and stomach yang deficiency syndrome,qi stagnation and blood stasis syndrome, liver and kidney yin deficiency syndrome.6.Correlation analysis of TCM syndrome types:There is a correlation between gender and TCM syndrome types. In male cases,damp-heat stasis syndrome and spleen deficiency and dampness stagnation syndrome are more common, followed by spleen and stomach Yang deficiency syndrome;female cases are damp-heat stagnation syndrome and spleen-stomach yang syndrome.Deficiency syndrome is more common, followed by Qi stagnation and blood stasis syndrome. There is a correlation between BMI value, exercise volume and TCM syndrome type. The TCM syndrome type of obesity and low exercise volume is mainly damp-heat stasis syndrome. There is a correlation between the number of incidences and the types of TCM syndromes.Compared with the initial cases, there are more cases of recurrence with the syndrome of yang deficiency in the spleen and stomach. There is no obvious correlation between the performance under colonoscopy and the TCM syndrome type.Pathological classification is related to TCM syndrome types.Non-adenomatous polyps are more common in damp-heat stasis syndrome, spleen deficiency and dampness syndrome,qi stagnation and blood stasis syndrome;adenomatous polyps are more common in spleen deficiency dampness stagnation syndrome,damp-heat stasis syndrome,Spleen and stomach yang deficiency syndrome. Conclusion:There are more men with colorectal polyps than women. The age of onset is more common in the elderly, and the recurrence rate increases with age.The number of polyps is mainly multiple, the size is mainly<0.5cm,and the shape is more common in Yamada I.The incidence is more common in the sigmoid colon and rectum, and the proportion of tubular adenomas is the highest in the pathological examination.Obesity, low physical activity, smoking, drinking, and abnormal blood lipid levels are related to the onset of polyps.Smoking, drinking, and reduced HDL-C levels will increase the risk of adenomatous polyps. The distribution of TCM syndrome types is most common in damp-heat stasis syndrome, spleen deficiency and damp stagnation syndrome.Male cases have more syndromes of damp-heat and stasis syndrome and spleen deficiency and dampness stagnation syndrome than females;female cases have more syndromes of qi stagnation and blood stasis than males. The TCM syndromes of obesity and low exercise are mainly damp-heat and stasis syndrome.Spleen and stomach yang deficiency syndrome is more common in recurrence cases than the initial case.Non-adenomatous polyps are more common in damp-heat stasis syndrome, spleen deficiency and dampness syndrome,qi stagnation and blood stasis syndrome;adenomatous polyps are more common in spleen deficiency and dampness stasis syndrome,damp-heat stasis syndrome,spleen-stomach yang deficiency syndrome. There is no obvious correlation between the performance under colonoscopy and the TCM syndrome type. |
