| Objective:This study is based on the spleen and kidney related theories,by observing the clinical symptoms,the improvement of uterine artery resistance,serum sex hormone levels and the embryo development in patients with early threatened abortion of spleen and kidney deficiency type by using Hutaiyin combined with dydrogesterone.To investigate the clinical effect of Hutaiyin in patients with early threatened abortion of spleen and kidney deficiency type,and to provide a new diagnostic thought for the clinical prevention and treatment of threatened abortion.Method:A total of 60 patients with early threatened abortion who came to Nanjing Hospital of Chinese Medicine for outpatient or inpatient treatment from March 2019 to January 2021 meeting the diagnostic criteria are selected.All the subjects are randomly enrolled into the control group(dydrogesterone tablet,10mg,q8h,oral)and the treatment group(dydrogesterone tablet,10mg,q8h,oral+Hutaiyin,200ml,100ml in the morning and evening,warm),with 30 cases in each group.The continuous treatment lasted for 4 weeks as a course of treatment,and one course of treatment is observed.Observing two groups of patients before and after treatment of TCM syndrome integral,uterine artery blood flow resistance index(RI),pulse index(PI),the peak systolic velocity/end-diastolic velocity(S/D)levels of change and the embryonic development situation,and serum estradiol(E2),progesterone(P)and human chorionic gonadotropin(dynamic changes of beta HCG)level to evaluate the clinical effect of Hutaiyin on early threatened abortion with spleen and kidney deficiency.Result:1.TCM syndrome scores:The scores statistically analyzed of the two groups are significantly lower than before treatment(P<0.01).The score of the treatment group is lower than that of the control group after treatment,and the difference is statistically significant(P<0.05).It shows that the treatment group has advantages in improving clinical symptoms.2.Uterine artery blood flow:After treatment,RI,PI,S/D values of uterine artery in both groups are significantly lower than those before treatment by statistical analysis(P<0.01).There is no significant difference in S/D between the two groups after treatment(P>0.05).RI and PI of the treatment group are lower than those of the control group,with statistical significance(P<0.05).This indicates that the treatment group has more advantages in improving the blood circulation of the uterus and placenta.3.Serum pregnancy hormone:After treatment,serum levels of E2,P and β-HCG in 2 groups were significantly higher than those before treatment(P<0.01).It can be seen from the line chart,serum levels of E2,P and β-HCG in 2 groups increased with the increase of gestational age,but the increase rate in treatment group was significantly higher than that in control group.After treatment,there was no difference in serum P level between the two groups(P>0.05).Serum E2 and β-HCG levels in the treatment group were higher than those in the control group,and the difference was statistically significant(P<0.05).It suggested that the treatment group had more advantages in improving serum pregnancy hormone.4.The curative effect and comprehensive curative effect of TCM syndromes in the treatment group reached 93.33%,and the control group reached 86.67%.The therapeutic effect of the treatment group is better than that of the control group,and the difference is statistically significant(P<0.05).5.Safety index:From the beginning of medication to the end of the course of treatment,no toxic and side effects occurred in 60 subjects.There is no abnormality in the safety-related inspection indicators.Conclusion:Hutaiyin can improve the clinical symptoms of patients with early threatened abortion of spleen-kidney deficiency,reduce uterine artery resistance,improve placental blood perfusion,and increase the level of pregnancy-related hormones.In addition,there are no toxic side effects during medication,and has high safety coefficient... |
