Objective: Based on coronary intravascular ultrasound technology,we have obtained clinical evidence that Qishen Yiqi dripping pills improve the stability of coronary plaques,providing new evidence support and application value for Chinese medicine in the inhibition of plaque rupture,and preliminary molecular level Explore its relationship with plasma miR-155,and provide new clinical data for traditional Chinese medicine in the field of cardiovascular disease prevention and treatment.Methods:Included 62 inpatients diagnosed with unstable angina in the Cardiovascular Department of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine from January 2019 to February2020,including 31 cases in the control group,31 cases in the treatment group,and the control group Western medicine treatment,the treatment group was combined with Qishen Yiqi dripping pills on the basis of Western medicine treatment,and the treatment course of both groups was 12 months.The coronary plaque changes of the two groups of patients were evaluated by IVUS examination indicators.The control group and the observation group were compared in the smallest lumen area(MLA),extravascular elastic membrane area(EEMA),plaque area(PA),and plaque load(Differences in PB),eccentricity index(EI),remodeling index(RI),fibrous cap thickness(FCT),maximum plaque thickness(Max PT),minimum plaque thickness(MPT),lipid pool area(LPA),And the differences in the results of angina pectoris efficacy,TCM syndrome efficacy,related laboratory indicators,echocardiogram,and relative expression of miR-155 between the two groups.All clinical data were processed using SPSS 22.0 software to complete statistical analysis,and all data results were statistically significant at P<0.05.Results:1.There is no statistically significant difference between the control group and the observation group in terms of clinical general baseline data(P>0.05).2.Before treatment,there was no significant difference between the control group and the observation group in IVUS examination indexes,TCM syndrome scores,related laboratory indexes,echocardiogram,and relative expression of miR-155(P>0.05).3.Comparison of IVUS indicators: After treatment,EEMA,MLA,PA,PB,FCT,RI,LPA,Max PT,MPT were all improved compared with before treatment(P<0.05);there was no significant difference in EI before and after treatment in the control group(P>0.05).EI in the observation group was significantly lower after treatment than before treatment(P<0.05),there was no significant difference in EI between the two groups after treatment(P>0.05);the observation group was in MLA,PA,PB,Max PT after treatment Compared with the control group(P<0.05);after treatment,the two groups had no significant difference in EEMA,FCT,RI,EI,LPA,MPT comparison(P>0.05).4.The curative effect of angina pectoris: After treatment,the total effective rate of the control group was 63.33%,and the total effective rate of the observation group was 96.77%.The improvement of the curative effect of angina in the observation group was better than that of the control group(P<0.05).5.In terms of the efficacy of TCM syndromes: After treatment,the TCM syndrome scores of the two groups were significantly lower than those before treatment(P<0.05).After treatment,the TCM syndrome scores of the observation group were lower than those of the control group(P<0.05);The total effective rate of TCM syndromes in the control group was 83.33%,and the total effective rate in the observation group was 96.77%.The number of markedly effective cases in the observation group was significantly higher than that in the control group.The difference in the efficacy of TCM syndromes between the two groups was statistically significant(P<0.05),there was no significant difference in the total effective rate between the two groups(P>0.05).6.Relevant laboratory indicators: After treatment,the blood lipid levels(TC,TG,LDL-c)of the control group and observation group were lower than before treatment(P<0.05);there was no significant difference in the control of TC levels between the two groups after treatment Significance(P>0.05),the TC levels of the two groups of patients were controlled well;the control of TG and LDL-c in the observation group after treatment was better than that of the control group(P<0.05);the CRP levels of the two groups of patients after treatment were higher than those before treatment Significantly lower(P<0.05).The CRP level of the observation group was significantly lower than that of the control group(P<0.05).7.Echocardiographic evaluation of cardiac function: EF and SV of the observation group improved compared with the control group after treatment(P<0.05),and LVESd of the observation group decreased compared with the control group(P<0.05);there was no significant difference in LVEDd between the two groups after treatment(P>0.05).8.Comparative analysis of the relative expression of miR-155: The relative expression of miR-155 in the control group was not significantly different before and after treatment(P>0.05),and the relative expression of miR-155 in the observation group after treatment was higher than before treatment(P<0.05);After treatment,the relative expression level of miR-155 in the observation group was higher than that in the control group(P<0.05).Conclusion: Qishen Yiqi Dripping Pills can regulate inflammatory reactions and blood lipid metabolism,stabilize coronary plaques,inhibit the occurrence and development of plaques,improve heart function,and reduce clinical symptoms.The mechanism may be related to the regulation of the relative expression level of miR-155.
