Objective:This study aims to analyze the correlation between parameter of thromboelastography and hypertensive intracerebral hemorrhage hematoma expansion,to find the predictive TEG parameters of hematoma enlargement.This study aims to analyze the correlation between the parameters of the thromboelastogram and the expansion of hematoma in hypertensive cerebral hemorrhage within 24 hours,and to explore the parameters of thromboelastography wether can predict the expansion of the hematoma.Methods:This is a retrospective study of hypertensive intracerebral hemorrhage patients admitted to our intensive care unit,which included four wards with 126 beds,between January 2019 and December 2019.The clinical data such as TEG,conventional coagulation,blood routine,and other laboratory indicators,and head CT on admission,a swell as re-examination of head CT within 24 hours after admission were collected by us.The Computer-assistedvolumetric nalysis method is used to calculate the amount of cerebral hemorrhage,when the patient was admitted to the hospital and within 24 hours after admission.According to whether the amount of cerebral hemorrhage within 24 hours of admission more than 2 m L or33% growth compared to the the amount of cerebral hemorrhage at admission,we divided the patients into the hematoma expansion group and the without hematoma expansion group.All analyses were conducted using SPSS,V.26.0(IBM).The variables index significantly associated with hematoma expansion in the univariate analysis were used in the correlation analysis.Multivariate binary logistic regression analysis was used to get the independent factors for hematoma expansion within 24 hours of hypertensive intracerebral hemorrhage.Plotting ROC curve analysed the predictive value of TEG parameters.α=0.05 was considered significant.Results:1.Univariate analysis found significant differences in the proportion of abnormal MA values on admission,platelet count and thrombocytocrit between the two groups(P<0.05).Correlation analysis found that the MA value on admission was positively correlated with the hematoma expansion of patients with hypertensive intracerebral hemorrhage within 24 hours,however platelets and thrombocytocrit were negatively correlated with hematoma expansion(P<0.05).2.Multivariate binary logistic regression analysis showed that the MA on admission was independent factors for hematoma expansion within 24 hours after admission of patients with hypertensive intracerebral hemorrhage(P<0.05).3.The area under ROC curve of MA value on admission was 0.68,the 95% confidence interval is 0.58 to 0.78,the sensitivity was 40.00%,and the specificity was 78.50%(P<0.05).the sensitivity is 57.90 %,the specificity is 78.50%(P<0.05).Conclusion:The abnormal MA value in TEG parameters on admission,which was positively correlated with the hematoma expansion in patients with hypertensive intracerebral hemorrhage within 24 hours,was an independent risk factor for hematoma expansion within 24 hours.Abnormal value on admission can be used as an independent predictor for hematoma expansion of patients with hypertensive intracerebral hemorrhage within 24 hours.
