Objective:This study observes the clinical efficacy of gubenbaotai decoction combined with progesterone in the treatment of early threatened abortion of kidney deficiency type by case-control study method,and explores its mechanism of action,so as to provide more basis and better diagnosis and treatment methods for clinical treatment of early threatened abortion of kidney deficiency type.Methods:Following the case-control study method,the data of patients with early threatened abortion of kidney-deficiency type were statistically analyzed.66 patients with early threatened abortion of kidney-deficiency type were selected and randomly divided into treatment group(oral gubenbaotai decoction plus intramuscular progesterone injection)and control group(oral baotailing tablets plus intramuscular progesterone injection)with 33 cases each group.During the treatment,2 patients dropped out in the treatment group,1 patient terminated,and 1 patient dropped out in the control group.Two groups of patients were treated until 12 weeks of gestation.The syndromic effective rate and total effective rate of the two groups of patients after treatment were observed respectively.The syndrome score,serum progesterone(P),β-human chorionic gonadotropin(β-hCG),estradiol(E2),cancer antigen 125(CA125),homocysteine(Hcy),immune cytokine interferon-y(IFN-y)and immune cytokine interleukin(IL)were observed before treatment,7 days and 14 days after treatment.The levels of IL-10 and the ratio of Th1/Th2(IFN-y/IL-10)cytokines were measured.Results:The comprehensive efficiency of the treatment group and the control group reached 96.67%and 78.12%,respectively.The treatment group was superior to the control group,and the difference was statistically significant(P<0.05).The total effective rates of TCM syndromes in the treatment group and the control group were 96.67%and 84.37%,respectively,with a statistically significant difference(P<0.05).After 7 days and 14 days of treatment,the scores of TCM syndromes in both groups were decreased compared with those before treatment,and the differences between the two groups were significant(P<0.01).At the same time after treatment,the scores of TCM syndromes in the treatment group were significantly lower than those in the control group,with a statistically significant difference(P<0.01).After 7 days and 14 days of treatment,the serum levels of beta-HCG,P and E2 in both groups were increased compared with those before treatment,and the differences between the two groups were significant(P<0.01).At the same time after treatment,the serum levels of beta-HCG,P and E2 in the treatment group were significantly higher than those in the control group,with a statistically significant difference(P<0.05).After 7 days and 14 days of treatment,the serum levels of CA125 and Hcy in both groups were decreased compared with those before treatment,and the differences between the two groups were significant(P<0.01).At the same time after treatment,the serum levels of CA125 and Hcy in the treatment group were significantly lower than those in the control group,with a statistically significant difference(P<0.05).After 7 days and 14 days of treatment,the serum levels of IFN-gamma in both groups were decreased compared with those before treatment,and the differences between the two groups were significant(P<0.01).At the same time after treatment,the serum levels of IFN-gamma in the treatment group were significantly lower than those in the control group,with a statistically significant difference(P<0.01).After 7 days and 14 days of treatment,the serum levels of IL-10 in both groups were increased compared with those before treatment,and the differences between the two groups were significant(P<0.01).At the same time after treatment,the serum levels of IL-10 in the treatment group were significantly higher than those in the control group,with a statistically significant difference(P<0.01).After 7 days and 14 days of treatment,the levels of Thl/Th2(IFN-y/IL-10)in both groups were decreased compared with those before treatment,and the differences between the two groups were significant(P<0.01).At the same time after treatment,the levels of IFN-γ/IL-10 in the treatment group were significantly lower than those in the control group,with a statistically significant difference(P<0.01).Conclusion:The treatment of early threatened abortion with gubenbaotai decoction combined with progesterone can significantly improve the TCM syndrome manifestation of this type of patients,and decrease or increase the test indicators,with significant clinical efficacy.It opens up new treatment ideas for treating early threatened abortion of kidney deficiency type.
