Objective:Objective to investigate the status of serum magnesium level,mineral metabolism and renal anemia in peritoneal dialysis patients in our hospital,analyze the relationship between serum magnesium level and mineral metabolism such as calcium,phosphorus,parathyroid hormone and renal anemia,and explore the influencing factors of serum magnesium level in peritoneal dialysis patients,so as to provide theoretical basis for intervention of serum magnesium level and diagnosis and treatment of complications in peritoneal dialysis patients.Methods:A total of 150 patients with peritoneal dialysis in our hospital from January 2019 to December 2020 were retrospectively analyzed.First,according to the normal range of serum magnesium concentration in our hospital is 0.75-1.02mmol/L,the patients were divided into hypomagnesemia group:Mg2+<0.75mmol/L;normal blood magnesium group:0.75≤Mg2+≤1.02mmol/L;hypermagnesemia group:Mg2+>1.02mmol/L;blood pressure,height,weight and body mass index were collected.Fasting elbow venous blood was collected in the morning to measure the following indexes:blood magnesium,blood calcium,blood phosphorus,and parathyroid hormone,i PTH,urea nitrogen,serum creatinine,triglyceride(TG),total cholesterol(TC),low density lipoprotein(LDL)and high density lipoprotein(HDL)Serum albumin,C-reactive protein,hemoglobin,hematocrit,folic acid,vitamin B12,serum ferritin,hypertension and diabetes history were analyzed.Correlation test was used to analyze the correlation between serum magnesium and general clinical data and laboratory indicators,and ordinal logistic regression was used to analyze the significant correlation factors of serum magnesium level.Results:A total of 150 peritoneal dialysis patients were included in this study,of which 63.3%were male and 36.7%were female.The average age of patients who started peritoneal dialysis was(57.87±15.14)years old,and the average dialysis age was(8.25±7.16)months.Among them,23 cases(15.33%)had hypomagnesemia,92cases(61.33%)had normal blood magnesium and 35 cases(23.33%)had hypermagnesemia.The main cause of end-stage renal disease was chronic glomerulonephritis(58%),followed by diabetic nephropathy(27.33%).1.The relationship between serum magnesium and general clinical data:urea nitrogen,serum albumin,triglyceride index comparison,the difference was statistically significant(P<0.05);2.The relationship between serum magnesium and mineral metabolism:potassium,sodium,phosphorus,i PTH,calcium phosphorus product index comparison,the difference was statistically significant(P<0.05);but blood calcium,corrected calcium index comparison,the difference was not statistically significant(P<0.05);3.The relationship between serum magnesium and anemia index:there were significant differences in hemoglobin,serum iron and serum ferritin(P<0.05);4.The results of correlation analysis showed that serum magnesium was positively correlated with serum creatinine(r=0.189,P<0.05),urea nitrogen(r=0.206,P<0.05),serum albumin(r=0.206,P<0.05),triglyceride(r=0.336,P<0.001),serum potassium(r=0.359,P<0.001),serum phosphorus(r=0.217,P<0.05),calcium-phosphorus product(r=0.219,P<0.05),hemoglobin(r=0.303,P<0.001).Was negatively correlated with sodium(r=-0.310,P<0.001)and i PTH(r=-0.219,P<0.05);5.Logistic regression results showed that triglyceride,potassium,phosphorus,calcium phosphorus product,i PTH,sodium and hemoglobin were significantly correlated with blood magnesium level.Conclusions:1.Both hypermagnesemia and hypomagnesemia occurred in peritoneal dialysis patients,which indicated that magnesium metabolism disorder occurred in patients with end-stage renal disease;2.Serum magnesium is correlated with potassium,phosphorus,i PTH,calcium and phosphorus product,which indicates that magnesium level is related to mineral metabolism and provides a new therapeutic target for comprehensive management of calcium and phosphorus metabolism;3.Patients with hypermagnesemia usually have higher levels of albumin and hemoglobin than those with hypomagnesemia,and their nutritional status is good,so the blood magnesium level can reflect the nutritional level to a certain extent.
