| Objective:To evaluate the clinical effect of Taohong Danggui Yinzi on skin pruritus of hemodialysis patients with blood deficiency,wind dryness and blood stasis,and to explore the possible mechanism of Taohong Danggui Yinzi in treating skin pruritus of hemodialysis patients with blood deficiency,wind dryness and blood stasis by observing the relief of Taohong Danggui Yinzi on skin pruritus of hemodialysis patients with blood deficiency,wind dryness and blood stasis,as well as the effects on VAS score,5-D pruritus score,hs-CRP,IL-6,hemoglobin and other indicators.Methods:70 patients of uremic pruritus with blood deficiency,wind dryness and blood stasis were selected according to the inclusion criteria and exclusion criteria.The patients were randomly divided into the treatment group and the control group by random number table method,35 cases in each group.In addition,20 healthy people in the physical examination center of our hospital were selected as the healthy group.On the basis of routine treatment,the control group was given loratadine orally,while the treatment group was given Taohong Danggui Yinzi orally,and the course of treatment was 8 weeks.TCM symptom score,VAS score,5-D pruritus score,serum creatinine(Scr),blood urea nitrogen(BUN),blood calcium(Ca),blood phosphorus(P),parathyroid hormone(PTH),blood β2 Microglobulin(β2-MG),high sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),hemoglobin(HGB)changes were observed and recorded before and after treatment,and carry out related statistical analysis.Results:1.Before treatment,serum hs-CRP and IL-6 in uremic pruritus group were significantly higher than those in healthy group(P<0.01),and the difference was statistically significant.2.Before treatment,the general data(gender,age,dialysis age,primary disease)of treament and control group,TCM symptom score,VAS score,5-D pruritus score,hs-CRP,IL-6,HGB and other clinical test indicators were not statistically significant(P>0.05)and is comparable.3.VAS scores were positively correlated with hs-CRP(r=0.682,P < 0.01)and IL-6(r=0.490,P<0.01),and the correlation was statistically significant;5-D pruritus scores were positively correlated with hs-CRP(r=0.622,P<0.01)and IL-6(r=0.532,P<0.01),and the correlation was statistically significant.4.After treatment,the VAS score and 5-D pruritus score of the control group and the treatment group decreased compared with those before treatment,and the degree of decrease in the treatment group was more obvious than that in the control group,and the difference was statistically significant(P<0.05).5.After treatment,the levels of blood P,blood PTH,and blood β2-MG in the control group and the treatment group were lower than those before treatment,and the blood Ca level was higher than that before treatment,and the treatment group was better than the control group.The difference was statistically significant(P<0.05).6.After treatment,the level of HGB in both groups increased compared with that before treatment,and the increase level in the treatment group was more obvious than that in the control group.The difference was statistically significant(P<0.05).7.After treatment,the blood levels of hs-CRP and IL-6 in the control group and the treatment group decreased compared with those before treatment,and the treatment group was even lower than the control group.The difference was statistically significant(P<0.05).8.After treatment,the TCM syndrome scores of the control group and the treatment group decreased,and the treatment group was even lower than the control group.The difference was statistically significant(P<0.05).Conclusion:1.The levels of hs-CRP and IL-6 in the treatment group and the control group were significantly higher than those in the healthy group,suggesting that the patients with skin pruritus on maintenance hemodialysis have micro-inflammation.In addition,hs-CRP and IL-6were closely related to the degree of pruritus in UP patients,which can be used as evaluation indexes for the effectiveness of pruritus in UP patients.2.Taohong Danggui Yinzi can effectively relieve skin pruritus symptoms of hemodialysis patients with blood deficiency,wind dryness and blood stasis.3.Taohong Danggui Yinzi can reduce the blood hs-CRP,IL-6,blood P,PTH and bloodβ2-MG levels of patients with hemodialysis skin pruritus of the type of blood deficiency,wind dryness and blood stasis,and increase the blood Ca level.It can improve the micro-inflammation state and calcium and phosphorus metabolism problems to a certain extent,and the curative effect is better than that of western medicine alone.4.Taohong Danggui Yinzi can improve the HGB level of patients with skin pruritus after maintenance hemodialysis,and can promote anemia to a certain extent.5.In the course of this study,no obvious side effects were found in Taohong Danggui Yinzi,which is safe and reliable,and has good clinical application value. |
