| Objective:To investigate the relationship between PTX3,CRP and the severity and prognosis of elderly patients with acute cerebral infarction(ACI).Method:According to the criteria of inclusion and exclusion,120 elderly ACI patients who were visited in general medical department,neurology department and emergency department were selected,and the serum PTX3 and CRP levels were measured.According to the National Institute of Health Stroke Scale at the time of admission,they were divided into two groups,among which NIHSS score less than 5was classified as mild cerebral infarction group,NIHSS score ≥ 5 was classified as moderate and severe cerebral infarction group,and the serum PTX3 and CRP levels of the two groups were compared;based on Barthel Index score measured 60 days after discharge from the study object,they were divided into two groups,among which BI index score ≤ 60 was classified as poor prognosis group,BI score > 60 was classified as good prognosis group,and the difference between PTX3 and CRP levels in serum of the two groups was compared.Spearman correlation analysis was used to analyze the relationship between PTX3 and CRP levels in blood clear of the study object and the severity and prognosis of the subjects.The subjects were used to analyze the relationship between PTX3 and CRP levels in the blood clear of the study subjects and the severity and prognosis of the subjects The results showed that the PTX3 and CRP levels were evaluated by ROC curve,and the high predictive value was selected.Result:1.According to NIHSS score,the levels of PTX3 and CRP in mild cerebral infarction group(average 7.43(2.89)ng / m L,4.70(3.70)mg / L)were significantly lower than those in moderate and severe cerebral infarction group(average 9.84 ±2.03 ng / m L,8.42 ± 2.86 mg / L),the differences were statistically significant(P =0.000,0.000);2.According to BI index score,the levels of PTX3 and CRP in poor prognosis group(average 9.75 ± 2.25ng/ml,7.89 ± 2.70 mg / L)were significantly higher than those in good prognosis group(average 7.54(3.24)ng / ml,4.80(5.15)mg / L),the differences were statistically significant(P = 0.001,0.011);3.Spearman correlation analysis showed that PTX3 and CRP levels were positively correlated with NIHSS score(correlation coefficients were 0.513 and 0.456,P = 0.000,0.000),and negatively correlated with Bi index score(correlation coefficients were-0.459 and-0.375,P = 0.000,0.000);4.ROC curve showed that the levels of PTX3 and CRP had good predictive value for the severity of the disease(area under the curve was 0.760 and 0.730respectively);the levels of PTX3 and CRP also had good predictive value for the prognosis of the patients(area under the curve was 0.696 and 0.644 respectively);and the predictive value of PTX3 was better than that of CRP,with statistical significance(P = 0.000,0.000).Conclusion:1.The more serious the patients were,the higher the serum levels of PTX3 and CRP were;the higher the levels of PTX3 and CRP were,the lower the self-care ability and the worse the prognosis were 60 days after discharge.Therefore,the levels of PTX3 and CRP should be detected in the early stage of the disease in order to evaluate the disease early;2.Both PTX3 and CRP can predict the severity and prognosis of ACI in elderly patients.In contrast,PTX3 has higher predictive value. |
