Abstract/Summary:
BackgroundStudies have shown that the abnormal expression of Fibroblast Growth Factor 23(FGF23)can play an important role in the pathogenesis of coronary atherosclerosis(Atherosclerosis,AS)by mediating vascular endothelial cell dysfunction. Then induce and promote the occurrence and development of Acute Coronary Syndrome(ACS).But whether the expression level of FGF23 is related to the severity of coronary artery stenosis, there is no relevant research.ObjectiveBy comparing the level of FGF23 in patients with acute coronary syndrome and healthy people, and the difference in the expression level of FGF23 in patients with different severity of coronary artery disease,to explore the correlation between FGF23 and the severity of coronary artery stenosis.MethodsFrom November 2019 to March 2020,101 patients who attended the Second Department of Cardiology of the Third Affiliated Hospital of Xinxiang Medical College and were clearly diagnosed with ACS by a cardiologist were selected and included in the ACS group, and based on the results of coronary angiography,Using the internationally accepted visual diameter method(degree of stenosis(%)=(D-d)/D×100%,D is the diameter of the reference vessel,d is the diameter of the narrowest part of the stenosis),of which ≥50% is considered clinically significant Positive lesions. At the same time,73 healthy patients in the physical examination center of our hospital during the same period were selected as the control group.Collect general data and peripheral blood of all research subjects, and uniformly determine biochemical indicators: creatine kinase(CK),high-sensitivity C-reactive protein(Hs-CRP),glucose(Glu),total cholesterol(CHOL),triglycerides(TG),high-density lipoprotein cholesterol(HDL),low-density lipoprotein cholesterol(LDL),homocysteine(HCY),FGF23,NO,and endothelin 1(ET-1)levels,statistics of patients with positive coronary artery disease The GENSINI score.By comparing the expression level of FGF23 in peripheral blood between the ACS group and the healthy control group, and the relationship with the GENSINI score,to analyze the correlation between FGF23 and the severity of coronary artery stenosis.Result1.The level of peripheral blood FGF23 in the acute coronary syndrome group was significantly higher than that in the healthy control group(P<0.05).2.Correlation analysis showed that FGF23 was positively correlated with GENSINI score(P<0.01).3.In addition, correlation analysis showed that NO in the acute coronary syndrome group was significantly lower than that in the healthy control group(P<0.05),and the levels of CK and ET-1 in the acute coronary syndrome group were higher than those in the healthy control group(both P <0.05).And the correlation analysis showed that FGF23 was negatively correlated with NO(P<0.01),and positively correlated with CK and ET-1(both P<0.01).Conclusion1.Increased levels of FGF23 in peripheral blood may indicate the possibility of coronary artery disease in patients.2.The expression level of FGF23 is positively correlated with the severity of coronary artery stenosis, and FGF23 can reflect the degree of coronary artery stenosis to a certain extent.3.FGF23 can reduce the synthesis of NO in coronary endothelial cells and increase the synthesis of ET-1,which in turn leads to vascular endothelial dysfunction.
