Objective: This experiment through early detection Bregs of CD19 + B cells and cytokine IL-2,IL-6,IL-10,IFN-γ,and TNF-α in relatively low and high risk MDS patients and healthy controls to explore Bregs and cytokines in immune pathogenesis of MDS.Method s: A total of 28 newly diagnosed MDS patients in the Department of He ma to logy of Lanzhou University Second Hospital from Jun 2019 to Jan 2021 were collected,including 18 in the low-risk group,10 in the high-risk group,and 10 in the heal thy control group.the proportion of peripheral blood Bregs cells in CD19+B cells of the healthy control group with low risk and high risk were detected by Flow cy to me try.The levels of cytokines I L-2,I L-6,I L-10,I F N-γ and T N F-α in peripheral blood were detected by CBA,and the correlation between Bregs and cytokines was further analyzed.Results: The level of Bregs in the high-risk group of MDS was significantly higher than that in the healthy control group and the low-risk group of MDS,and the low-risk group of MDS was lower than that in the healthy control group,with statistical significance(P < 0.001).The cytokine test results showed that the levels of IL-6,IL-10,IFN-γ and TNF-α in the low-risk group were significantly higher than those in the healthy control group(P<0.001).The levels of IL-2,IFN-γ and TNF-α in MDS highrisk group were significantly lower than those in MDS low-risk group(P<0.001).IL-10 level was significantly higher in MDS high-risk group than in MDS low-risk group(P<0.001).The levels of IL-2 and IFN-γ in healthy control group were significantly higher than those in high-risk MDS group(P<0.01).The levels of IL-6,IL-10 and TNF-α in healthy control group were significantly lower than those in MDS high-risk group(P<0.001).Correlation analysis showed that Bregs cells were negatively correlated with IL-2,IL-6,IFN-γ,TNF-α,and positively correlated with IL-10 in the low-risk group of MDS patients.Bregs cells in MDS patients with high risk group were negatively correlated with IL-2 and TNF-α,and positively correlated with IL-6 and IL-10.Further comparison with clinical parameters showed that Bregs level in peripheral blood of newly diagnosed MDS patients was negatively correlated with hemoglobin concentration,and had no correlation with neutrophils count and platelet count.Conclusion: 1.Abnormal expression of Bregs,IL-2,IL-6,IL-10,IFN-γ and TNF-α in different risk stratification MDS suggests that Bregs and other immune disorders may be involved in the pathogenesis and development of MDS.2.Bregs of MDS patients in the low-risk group were negatively correlated with cytokines IL-2,IL-6,IFN-γ,TNF-α,and positively correlated with IL-10,while Bregs of MDS patients in the high-risk group were negatively correlated with cytokines IL-2,TNF-α,and positively correlated with IL-6,IL-10.It is suggested that Bregs and cytokines may interact together to cause the immune microenvironment disorder of MDS.3.The level of Bregs in MDS patients was negatively correlated with hemoglobin concentration,suggesting that Bregs may play a certain role in the occurrence of anemia in MDS patients.
