Clinical Study Of Yinshan Danfang On Acute Gouty Arthritis Of Dampness And Heat Accumulation

Objective:To study the clinical effect of Yinshan Danfang prescription combined with Celecoxib capsule in the treatment of acute gouty arthritis with a damp and heat accumulation knot type and they effect on NLRP3 inflammatory corpuscles signaling pathway related proteins.Methods:There were 46 patients with acute gouty arthritis from Department of Rheumatology,Affiliated Hospital of Jiangxi University of TCM that selected for this study,and they were randomly placed into two groups,the control group and the study group were both 23 cases in it.The control group was given celecoxib capsule,0.2 g once,2 /d,take after meals;The study group was given Yinshan Danfang prescription on the basis of above treatment,one dose a day,in which the study course of these two groups were both one week.In the meantime,observing the condition changes of the patients in the two groups,assessing the patient’s VAS score at the beginning and the end of the experiment,and serum UA、 CRP and ESR、IL-1、IL-8 and NLRP3 inflammasome levels were detected and recorded before and after trea tment.To observe if there were any adverse reactions,If there was an adverse reaction,Then timely and correct processing.Results :1.The total effective rate of the study group was 78.26%,and 60.87% in the control group,the clinical efficacy of the study group was evidently better than that of the control group(p<0.05).2.The VAS score of the study group was evidently lower than that of the control group after treatment(P<0.05).3.There was no evidently changes of serum UA level in the control group(P>0.05),but the serum UA level in the study group was dramatic declined than that before treatment(P<0.05),and the serum UA level in the study grou p was evidently less than that in the control group after treatment(P<0.05).4.Serum CRP、 ESR levels in both groups were dramatic declined than those before treatment(P<0.05),and those in the study group were evidently lower than those in the control group after treatment(P<0.05).5.Levels of serum IL-1β、 IL-8 in the two groups were dramatic declined after treatment(P<0.05),and those in the study group were evidently lower than those in the control group after treatment(P<0.05).6.Levels of peripheral blood NLRP3 inflammatory corpuscles in the two groups were dramatic declined after treatment(P<0.05),and those in the study group were evidently lower than those in the control group after treatment(P<0.05).7.During the treatment period,there were no terrible things happened,except that two patients in the control group were con stipated,and symptoms were relieved after acupuncture at Tianshu 、Dachangshu、 Zusanli、Hegu points.Conclusion :1.The clinical effect of Yinshan Danfang prescription joint use celecoxib capsule in the treatment of AGA with dampness and heat obstruction syndrome was better than that of celecoxib capsule alone.2.The VAS score in the study group that using Yinshan Danfang prescription combined with celecoxib capsule treating the damp-hest type of AGA was significantly lower than that of control group with celecoxib capsule alone,which suggests that Yinshan Danfang prescription can treat diseases as a whole,and it can effectively improve the clinical symptoms and signs of gout patients,alleviate the progress of the disease.3.Yinshan Danfang prescription combined with celecoxib capsule oral treating AGA with damp-heat obstruction syndrome can more quickly alleviate the joint swelling and pain than using celecoxib capsule alone.4.It was significantly better that Yinshan Danfang prescription combined with celecoxib capsule oral treating AGA with damp-heat obstruction syndrome in reducing inflammatory reaction,reducing acute phase reactant(CRP、 ESR)level than that of the single use of celecoxib capsule,this suggests that Yinshan Danfang prescription can reduce acute phase reactant(CRP、ESR)in human body,that is,it has anti-inflammatory effect.5.The effect of reducing serum UA level that Yinshan Danfang prescription combined with celecoxib capsule treating AGA with damp-heat obstruction syndrome was better than that of celecoxib capsule alone,which sugges ts that Yinshan Danfang prescription could reduce serum UA level in human body.6.Yinshan Danfang prescription combined with celecoxib capsule oral treating AGA with damp-heat obstruction syndrome was significantly better than celecoxib capsule alone in reducing inflammatory factor level(IL-1β,IL-8,NLRP3 inflammatory corpuscles),this suggests that Yinshan Danfang prescription combined with celecoxib capsule can inhibit inflammatory response,inhibit the expression of NLRP3 inflammatory corpuscles,block the conduction of inflammatory corpuscles related signaling channel and inhibit the expression of IL-1β、 IL-8 inflammatory factors,so that it can control the progress of acute gouty arthritis.7.Both Yinshan Danfang prescription combined with celecoxib capsule and celecoxib capsule alone were not produced adverse reaction,which indicates that Yinshan Danfang prescription combined with celecoxib capsule treating AGA is safe and useful.
