Study On The Correlation Between Micro-inflammatory State,Nutritional Status And TCM Syndrome Scores Of Damp-heat Syndrome In Maintenance Hemodialysis Patients

Abstract/Summary:
ObjectiveThe research aims to discuss the correlation between the traditional Chinese medicine(TCM)syndrome score of dampness-heat syndrome in maintenance hemodialysis patients(MHPs)with micro-inflammatory state and nutritional status,in order to explore the material basis of MHPs’ damp-heat syndrome,and provide modern medical evidence for the precise differentiation of TCM syndromes.MethodsIn this experiment,MHPs in the hemodialysis center of Hospital of Chengdu University of TCM were collected by cross-sectional method and divided into dampness-heat syndrome group and non-dampness-heat syndrome group according to TCM syndrome differentiation.In the indicator detection,the TCM syndrome score of dampness-heat syndrome group was calculated by using the quantification table of chronic renal failure dampness-heat syndrome symptom grading.The tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were detected by double-antibody sandwich enzyme-linked immunosorbent assay(ELISA),the hypersensitive C-reactive protein(hs-CRP)were determined by immunoturbidimetry,the hemoglobin(Hb)was determined by colorimetric method,and the albumin(Alb)level was detected by bromocresol green method.Serum TNF-α,IL-6 and hs-CRP were used to observe the micro-inflammatory state of MHPs,and the nutritional status of patients was evaluated by MQSGA score combined with Alb and Hb.Finally,Excel and SPSS26.0 were used for data collation and statistical analysis,and the relationship between MHP nutritional status,micro-inflammatory state and dampness-heat syndrome was analyzed by inter-group comparison and single factor combined with multi-factor correlation.Results1.Among 89 patients on maintenance hemodialysis enrolled in the study,the dampness-heat syndrome cases accounted for 43.8%,patients with hs-CRP levels greater than 5 mg/L accounted for 51.7%,and MQSGA score indicated that 55.1% of the patients were malnourished.2.There were observably statistical differences in age,MQSGA score,Hb,hs-CRP and TNF-α between dampness-heat syndrome group and non-dampness-heat syndrome group of MHPs(P<0.05).Logistic regression analysis showed that age and serum TNF-α level were positively correlated with the occurrence of dampness-heat syndrome(P<0.01,OR > 1),indicating that the older the age and the higher the TNF-α level,the higher the incidence of dampness-heat syndrome.3.The TCM syndrome score of dampness-heat syndrome were positively correlated with MQSGA and TNF-α in MHPs(P<0.05).And the MQSGA score of MHPs was also positively correlated with the level of serum hs-CRP(P<0.01).Thus,the more severe the micro-inflammatory state and malnutrition of MHPs,the higher the TCM syndrome score of dampness-heat syndrome.Conclusion1.Micro-inflammatory state and malnutrition are widely present in MHP of our hospital,which promote and grow each other.We should attach great importance to it.2.Dampness-heat accumulation is not only a pathological product of end-stage renal disease(ESRD),but also an important factor in the aggravation of ESRD.Old age and high level of serum TNF-α are risk factors of dampness-heat syndrome of MHPs.3.The more severe the micro-inflammatory state and malnutrition of MHPs,the higher the TCM syndrome score of dampness-heat syndrome,and the more severe the corresponding clinical symptoms,indicating that micro-inflammatory state and malnutrition are closely related to dampness-heat accumulation in renal failure disease of TCM.
