Objective:To investigate the effect of modified Qiming granule individualized dynamic treatment of diabetic retinopathy(DR)and diabetic peripheral neuropathy(DPN),and to evaluate the effect difference between individualized dynamic treatment and fixed prescription treatment of DR and DPN.Methods: A total of 44 patients with DR complicated with DPN who met the research criteria were included in the multicenter,randomized,controlled method,and were randomly divided into the experimental group and the control group.The experimental group was given basic treatment + Qiming granule + disease differentiation medication in DPN + medication for partial/concurrent syndrome;the control group was given basic treatment + Qiming granule,and the treatment period was 24 weeks.The changes of HbA1 c,vision,fundus condition,neurological symptom and sign score(TCSS),pain scale score(VAS),electromyography,TCM syndrome score and other safety indexes before and after treatment were observed.Results:1.There was no significant difference in HbA1 c after treatment compared with before treatment in both groups(P > 0.05);there was no significant difference in the decrease of HbA1 c between the two groups before and after treatment(P>0.05).The total effective rate of HbA1 c reduction in experimental group(63.64%)was higher than that in control group(52.38%).2.The visual acuity after treatment was improved in both groups compared with before treatment(P <0.05);there was no significant difference in the improvement of visual acuity between the two groups before and after treatment(P > 0.05).3.The total effective rate of the improvement of fundus lesions in experimental group(81.82%)was higher than that in control group(76.19%).4.TCSS scores after treatment were lower than before treatment in both groups(P<0.05);there was no significant difference in the decrease of TCSS scores between the two groups before and after treatment(P > 0.05).5.VAS scores after treatment were lower than before treatment in both groups(P<0.05);there was no significant difference in the decrease of VAS scores between the two groups before and after treatment(P>0.05).6.The conduction velocity of superficial peroneal nerve and sural nerve after treatment were improved compared with before treatment in experimental group(P < 0.05),and the conduction velocity of superficial peroneal nerve after treatment was improved compared with before treatment in control group(P<0.05);there was no significant difference in the improvement of conduction velocity and action potential amplitude of superficial peroneal nerve,sural nerve and total peroneal nerve between the two groups before and after treatment(P >0.05).7.TCM syndrome scores after treatment were lower than before treatment in both groups(P < 0.05);there was no significant difference in the decrease of TCM syndrome scores between the two groups before and after treatment(P > 0.05);the total effective rate of improving TCM syndromes in experimental group(100%)was higher than that in control group(90.48%).Conclusion:1.Qiming granule has definite efficacy in treating DR complicated with DPN.2.The effect of modified Qiming granule individualized dynamic treatment on HbA1 c,vision,fundus lesions,neurological symptoms and signs,pain degree,electromyography and TCM syndromes in patients with DR complicated with DPN showed a trend of better than that of Qiming granule alone.
