Objective:To observe the intervention effect of Qiming Granule on serum inflammatory factors NF-κB,MCP-1 and TNF-α in patients with diabetic microangiopathy of Qi and yin deficiency type,and to explore its mechanism from the inflammatory mechanism.Methods: 80 patients with diabetic microangiopathy of Qi and yin deficiency type with ocular and renal damage were selected and randomly divided into two groups according to the random number table.40 cases in the control group were treated with basic hypoglycemic,hypotensive and lipid-lowering treatment,and 40 cases in the experimental group were combined with Qiming granules on the basis of the control group for 3 months.Before and after treatment,the improvement of visual acuity,fundus,renal function,TCM syndrome score,the changes of inflammatory factor NF-κB,MCP-1,TNF α,the fluctuation of blood pressure,blood glucose and blood lipid were observed,and the adverse reactions were recorded.Results:1.80 subjects meeting the criteria were included in this study,and 73 cases were finally completed.The baseline data of gender,age and course of disease of the two groups before treatment were compared,P > 0.05,which was comparable.2.Comparison of blood pressure,blood glucose and blood lipid between the two groups: there was no significant fluctuation in blood pressure level,hablc and blood lipid level before and after treatment in the two groups,P > 0.05,no statistical difference.3.The improvement of visual acuity and fundus lesions in the two groups: compared within the two groups,the visual acuity of the patients in the experimental group after treatment was significantly higher than that before treatment,and fundus hemorrhage and exudation were reduced compared with that before treatment,the difference was statistically significant(P < 0.05 and P < 0.01).In the control group,the fundus hemorrhage was alleviated(P < 0.05),but the fundus exudation was not significantly improved(P > 0.05).Compared with the control group,the improvement of visual acuity and fundus lesions in the experimental group was better than that in the control group(P < 0.05).4.Improvement of renal function indexes in the two groups: compared within the group,the level of 24 h UTP in the two groups after treatment was lower than that before treatment,P<0.01,with statistical difference;for e GFR,the experimental group increased after treatment compared with before treatment(P < 0.05),but the control group did not significantly improve(P > 0.05).Compared with the control group,the decrease level of 24 h UTP in the experimental group was better than that in the control group,P < 0.05,with statistical difference.However,there was no significant difference in the improvement of e GFR between the two groups(P > 0.05).5.Comparison of TCM syndrome score and curative effect between the two groups:within the group comparison,the TCM syndrome score of the two groups after treatment was significantly lower than that before treatment,P < 0.01,with significant statistical difference.Compared with the control group,the score of TCM syndrome in the experimental group decreased significantly(P < 0.05).The total effective rate of the experimental group was 83.8%,which was significantly better than 52.8% of the control group(P < 0.01).6.The changes of inflammatory factors in the two groups: compared within the two groups,the levels of serum NF-κ B,MCP-1 and TNF α in the two groups decreased after treatment,P > 0.05,the difference was statistically significant.Compared with the control group,the level of inflammatory factors in the experimental group decreased significantly after treatment,P values were 0.014,0.015,0.032,respectively,the difference was statistically significant.7.Safety evaluation: in the course of this trial,2 patients in the test group developed diarrhea,which improved spontaneously after 1 week of drug withdrawal,and no other adverse reactions occurred.Conclusion: Qiming granule has protective effect on diabetic microangiopathy of Qi and yin deficiency type by reducing the level of inflammatory factors.
