| 【Objective】In this study, patients with infertility of low prognosis who have undergone IVF-ET again(kidney-yin deficiency-liver stagnation-blood stasis type)are the subjects of observation. We collect relevant data and conduct statistical analysis.By observing the improvement of patients’ ovarian function and the clinical efficacy of patients’ natural pregnancy through multi-channel intervention of traditional Chinese medicine, this study provides clinical evidence for the treatment of patients with infertility of low prognosis who have undergone IVF-ET again with traditional Chinese medicine intervention to improve the patient’s ovarian function, increase the natural conception rate, and improve the TCM syndromes of kidney-yin deficiency-liver stagnation-blood stasis type.【Method】Sequentially collect patients who attended the Reproductive Center of Sichuan Provincial People’s Hospital from February 2019 to December 2019,and those who attended the Reproductive Center of West China Second Hospital of Sichuan University from January 2020 to February 2021.All patients are patients who meet the second or third category of Poseidon’s classification criteria and undergo IVF-ET again. And they again IVF-ET program are planned to Gn RHant program, such patients a total of 110 cases.a total of 55 patients in the treatment group went to the gynecological clinic of the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine, they were treated with multi-channel intervention of TCM(oral Zishen Zhuyun recipe + retention enema + auricular point application)and DHEA.A total of 55 in the control group was treated with DHEA,and the two groups were treated for three menstrual cycles. The treatment was able to observe the changes of reproductive hormone level(b FSH,b FSH/b LH,b E2,AFC),TCM syndrome score, natural conception status(natural conception rate, natural conception outcome, natural conception successful pregnancy time distribution, natural conception Poseidon classification, previous infertility types, infertility years, previous IVF frequency distribution),.Finally,SPSS 26.0 statistical software was used for data analysis.【Results】1.The comparison between the treatment group before treatment and after treatment proved that Chinese medicine could bidirectional regulation of patients’ b E2,reduce patients’ b FSH,b FSH/b LH(P<0.05).Compared with the control group, the treatment group was more effective in improving b E2,b FSH/b LH(P<0.05).Both the treatment group and the control group had significant effects on improving patients’ FSH.After 3 months of treatment, the number of AFC in the treatment group and the control group was increased compared with that before treatment(P < 0.05),but the treatment group was significantly better than the control group in improving the number of AFC after treatment(P < 0.05).2.The treatment group was better than the control group in improving the main syndrome score, secondary syndrome score, total syndrome score, recovery rate of single symptom(main syndrome/secondary syndrome),total effective rate of TCM syndrome and less menstrual volume(P<0.05).3.4 cases of natural pregnancy in the treatment group, accounting for 8% of the total number of patients in the treatment group;Among them,3 cases(6%)confirmed clinical pregnancy, which was better than the control group(P < 0.05).【Conclusion】1.Multichannel treatment of traditional Chinese medicine can bidirectional regulation of b E2 in low prognosis patients with IVF-ET,reduce patients’ b FSH,b FSH/b LH,and increase the number of AFC,improve patients’ reproductive endocrine and ovarian function.2.Multi-channel treatment of traditional Chinese medicine can improve the TCM symptoms of kidney-yin deficiency-liver stagnation-blood stasis type in low prognosis IVF-ET patients, improve the situation of low menstrual volume, and improve the patients’ quality of life.3.Multi-channel treatment of traditional Chinese medicine can improve the natural pregnancy rate of low prognosis patients with second IVF-ET,and improve the pregnancy outcome of patients with natural pregnancy. |
