| According to the data of National Bureau of statistics in 2019,China’s population aged over 65 has increased to 11.9%.According to the classification standard“Population Aging and Its Socio-economic Consequences” which determined by the United Nations,China has already entered the stage of serious aging.China’s aging situation is becoming increasingly severe,not only with a large base and rapid growth,but also with aging,disability and empty nest.The rapid increase of pension demand with a gradually weakened family pension function.Institutional pension will bear more social pension responsibilities.With the advocacy of the combination of medical treatment and maintenance,hospice care has become the top priority to improve the quality of institutional pension.However,according to the report of 2015 death quality index,China ranks 71,and the people’s understanding of hospice care only scored one point.The huge demand is in sharp contrast with the extremely low cognition.Hospice care needs to be promoted and applied urgently.As the promoter of domestic hospice care service,The "Regulations on Social Work Services of Pension Agencies" has clearly takes hospice care service as an important service content of social work in pension institutions.Studying the practice of hospice caring for social work is of constructive significance for promoting its own development.Based on the on-the-spot investigation and overall study of the project of "Road to Peace" in Chongqing Y nursing home,this paper uses participatory observation method and case analysis method to explore hospice care social work practice.Theoretically connect the “Holistic” care mode,hospice care and social work,and sort out the relationship between the ecosystem and social support system of the hospice elderly in the “Holistic” care mode;then discuss the basic framework of the service from the function,service form and service structure of hospice care social work;Launching social work intervention cases involving elderly hospice care in residential care homes,combining case analysis and the project as a whole to concluded the hospice care social work practice mode under the "Holistic" care model.Then explain the specific intervention mechanism of the social work practice "Holistic" care mode when intervening in the hospice care of the elderly.Studying the practice of hospice care social work under the “Holistic Care" model provides solutions to the problems of hospice care in China that have started late and unestablished suitable hospice care model.At the same time,the study of Chongqing Y nursing home is very representative and can also provide experience and follow-up information for the hospice care of Chongqing’s elderly care institutions. |
