Data Mining And Meta-analysis Of Traditional Chinese Medicine For Acne Treatment

Background:acne is a chronic inflammatory skin disease that occurs in hair follicles and sebaceous glands.The prevalence rate is 9.38%.The incidence rate of acne is 8.1%in China.Its clinical characteristics are black and white acne and papules,and then pustules and cysts,even nodules and cysts.In the process of the disease accompanied with seborrhea,it is a chronic course.Acne not only affects the beauty,but also brings psychological diseases to some extent.At present,the pathogenesis of acne is not completely clear.Because of the wide range of acne and the interference of environmental factors,acne is easy to recur and difficult to cure.Therefore,acne has been a hot field in dermatology.The etiology of acne can be summarized as wind,humidity,heat,blood stasis,depression and phlegm,and then it is related to human quality and environment.The disease position is described from lung,spleen,stomach,liver,kidney,Chong Ren and other aspects.The pathogenesis of acne is related to the fever of lung,damp heat of stomach and intestine,phlegm dampness of spleen deficiency,blood stasis block,deficiency of kidney yin,disjunction of emotion,and inflammation of liver fire.In addition to the reasons listed above,female patients are also closely related to liver and impulse.In the aspect of syndrome differentiation,heat,depression and phlegm should be identified first.Among them,heat is divided into wind heat,blood heat and damp heat;Depression includes poison depression and blood stasis;Phlegm refers to the spleen and stomach disorders,dysfunction,dampness and turbidity,phlegm accumulation and stagnation of skin.At present,the syndrome differentiation and treatment in clinic can be divided into six types:lung meridian wind heat,damp heat of intestine and stomach,stagnation of liver meridian,phlegm heat and stasis,deficiency of liver and kidney yin and disorder of Chong Ren.The acne with phlegm heat and stasis stasis is serious,and the principle of clearing heat,removing phlegm and activating blood circulation is the principle of treatment,and the clinical effect is good.Traditional Chinese medicine is an important part of traditional Chinese culture.The curative effect of traditional Chinese medicine is remarkable.Compared with western medicine,it has the advantages of high safety,less adverse reactions,and price,and long-term use.Because modern doctors have different understanding of acne,different differentiation or differentiation,the treatment methods are different,but the overall effect is significant but lack of standardized summary and combing,and clinical lack of large sample observation of therapeutic effect,which limits the development of traditional Chinese medicine in the treatment of acne.To solve these problems,we must make a standardized and scientific summary of the literature of traditional Chinese medicine for acne treatment,explore the law of its use and the deep-seated study of meta analysis of acne treatment,so as to provide the basis for the proper choice of medication in clinical treatment of acne,and benefit the patients.Objective:1.to collect and summarize the clinical randomized controlled trials of acne treatment in recent ten years,and to summarize the rule of internal medicine in acne treatment,in order to provide data support for the clinical treatment of acne.2.to evaluate the efficacy and safety of traditional Chinese medicine combined with traditional western medicine in acne treatment,and provide evidence-based medical evidence for the clinical application of the combination of traditional Chinese and Western medicine in the treatment of acne.Methods:1.based on the selection of Chinese Journal Full-text Database(CNKI)containing acne prescription,R language is used for frequency analysis,system cluster analysis and association rule analysis,statistics of Chinese medicine prescriptions in the literature,data analysis,data distribution types and rules,and the characteristics of the groups and drug use rules are excavated.2.computer search the databases of CNKI,Wanfang,VIP,CBM,PubMed,EMBASE,Cochrane Library,and collect the literature of randomized controlled trials of traditional Chinese medicine combined with western medicine in the treatment of acne,The paper uses Cochrane collaborative network system evaluation manual to evaluate the quality of the documents,extract the basic information of the documents,and use revman5.3 software to carry out meta analysis.The clinical effect of traditional Chinese medicine and Western medicine conventional therapy for acne is systematically evaluated and the effective comparison is made.Results:1.443 acne prescriptions were selected,involving 243 Chinese herbs.The frequency analysis,systematic cluster analysis and association rule analysis were conducted for drugs with frequency more than 30 and except licorice.The common drugs were Scutellaria,forsythia forsythia,Salvia miltiorrhiza,dandelion,honeysuckle,red peony,peony skin,radix rehmanniae,mulberry white skin and white flower snake tongue grass;The combination of four different drugs and 24 groups of drug pairs;The drug properties with frequency more than 30 were mainly "cold","warm","flat",while the drug taste was mainly bitter,sweet and spicy,and the main channel was foot Jueyin liver meridian.Among the main therapeutic effects of drugs with frequency more than 30,the heat clearing drugs accounted for 53.68%,phlegm relieving cough and asthma drugs 12.22%,and blood activating and stasis removing drugs 9.64%.2.23 literatures were selected according to the standard of row.The effective rate of traditional Chinese medicine combined with western medicine was rr=1.50,95%CI[1.41,1.59]was better than that of Western Medicine(P<0.05).Conclusion:1.the prescription of acne treatment is based on the principle of clearing heat,combining with the principle of regulating qi,activating blood circulation and removing blood stasis.The most widely used prescriptions are traditional Chinese medicine of clearing heat and phlegm;The most selected group was forsythia,loquat leaf,Scutellaria baicalensis and other antiphlogistic drugs.The rule of this prescription provides some data support for the new combination of prescription in the treatment of acne.2.Meta analysis suggested that the efficacy of traditional Chinese medicine combined with western medicine in acne treatment was better than that of the traditional western medicine alone.
