| Objective:To summarize and analyze the syndromes and medication rules of Professor Jin Zhe in treating diminished ovarian reserve(DOR),and provide ideas for the clinical diagnosis and treatment of DOR with traditional Chinese medicine.Methods:Collect the medical records of DOR patients in the outpatient department of Professor Jin Zhe from January 2019 to December 2020,extract relevant symptoms and Chinese medicine prescriptions of patients who meet the criteria for inclusion,and enter the data into the Traditional Chinese Medicine Inheritance Assist Platform V2.5(TCMISS V2.5)after standardized processing,which counts the frequency of related symptoms,the frequency of drugs,the four characters,the five tastes,the channel distributions of herbs,the core drug pair and the potential new prescriptions,etc.,analyzes the statistical results,and summarizes the clinical treatment of DOR syndrome characteristics and medication rules.Results:1.A total of 59 patients with DOR were included in this study,among whom the oldest was 39 years old and the youngest was 27 years old.The average age of the patients was 35.6 years old.The number of patients over 35 years old accounted for 66%of the total number,and a total of 41 patients had the need for pregnancy.After screening,177 prescriptions were recorded,involving 131 traditional Chinese medicines.2.Distribution of related syndromes:The distribution of related symptoms mainly include menstrual blood clot(67.80%),low menstrual flow(49.15%),infertility(45.76%),abdominal pain during menstruation(27.29%),dark menstruation(33.90%),and backache(32.20%),secondly are premenstrual chest distension(23.73%),fatigue(18.64%),difficulty falling asleep(16.95%),dry stool(16.95%),chills(16.95%),etc.The main characteristics of tongue and pulse are dark red tongue(64.41%),thin white coating of the tongue(83.05%)and smooth pulse(84.75%).3.Drug frequency,character and taste,channel tropism:177 prescriptions of traditional Chinese medicine involved a total of 131 flavors of drugs.The top 20 high-frequency drugs include:Atractylodis Macrocephalae Rhizoma(107),Cuscutae Semen(102),Epimedii Folium(93),Hordei Fructus Germinatus(90),Salviae Miltiorrhizae(84),Radix et Rhizoma Rhodiolae crenulatae(78),Taxilli Herba(76),Poria cocos(74),Asini Corii Colla(71),Dioscoreae Rhizoma(69),Paeoniae Radix Rubra(67),Caulis Spatholobi(66),Eucommiae Cortex(64),Lycii fructus(63),Curcumae Radix(63),Chuanxiong Rhizoma(62),Cyperi Rhizoma(61),Cervi Cornus Colla(59),Paeoniae Radix Alba(58),Ligustri Lucidi Fructus(54),mainly tonic,The nature and flavor are mainly warm(1344),flat(1313).sweet(2608),bitter(1692),and pungent(1124).Most of them belong to Liver meridian(2634),Kidney meridian(1783),Spleen meridian(1420).4.Core Drug Combination:179 groups of drug combinations model that appeared more than 32 times(20%)were obtained and 20 groups of drug-related combinations with confidence?0.9 are obtained,from which 13 groups of drug compatibility were selected,including Dioscoreae Rhizoma-Atractylodis Macrocephalae Rhizoma,Cuscutae Semen-Dioscoreae Rhizoma-Rhodiolae Crenulatae Radix ET Rhizoma-Atractylodis Macrocephalae Rhizoma,Dioscoreae Rhizoma-Hedysari Radix-Atractylodis Macrocephalae Rhizoma,Paeoniae Radix Rubra-Paeoniae Radix Alba,Paeoniae Radix Alba-Ligustri Lucidi Fructus-Paeoniae Radix Rubra,etc.5.New prescription:Under the condition that the correlation is 6 and the penalty is 2,based on the unsupervised entropy hierarchical clustering analysis,10 potential core combinations are obtained,such as:Aurantii Fructus_Semen Plantaginis_Dianthi Herba,Spatholobi Caulis_Salviae Miltiorrhizae Radix et Rhizoma_Epimedii Folium,Taxilli Herba_Poria_Chuanxiong Rhizoma,and so on.Further obtain 5 potential new prescriptions:?Hedysari Radix_Rhodiola Crenulatae Radixet Rhizoma_Asini Corii Colla_Epimedii Folium_Cervi Colla Cornus_Eucommiae Cortex,?Spatholobi Caulis_Salviae Miltiorrhizae Radix et Rhizoma_Epimedii Folium_Taxilli Herba_Lycopi Herba_Dipsaci Radix,? Taxilli Herba_Poria_Chuanxiong Rhizoma_Hedysari Radix_Dioscoreae Rhizoma_Cuscutae Semen,?Taxilli Herba_Rosae Chinensisi Flos_Chuanxiong Rhizoma_Lonicerae Flos_Sargentodoxae Caulis_Prunellae Spica.Conclusion:1.Most of the clinically collected DOR patients were over 35 years old,and most of them were in need of pregnancy.The syndrome of deficiency of kidney and liver,deficiency of kidney and blood stasis,deficiency of spleen and kidney were more common.2.The treatment of DOR tonifying the kindey essence as the principle.The main drugs are tonic drugs,and the medicine of invigorating the circulation of blood,drugs for soothing liver and qi,and drugs for clearing away heat and dampness are used.The main medicine is calm,and the main medicine is sweet,bitter,and pungent.3.Professor Jin's treatment of DOR is based on tonifying kidney and filling essence,nourishing blood and promoting blood circulation.At the same time,it is combined with syndrome differentiation,with methods of promoting blood circulation to remove blood stasis,dredging liver and regulating qi,clearing heat and clearing damp,etc.Drugs mainly belong to liver,kidney and spleen meridian.The medicinal properties are mainly warm and calm,and the taste is mainly sweet,bitter and acrid.The medication is flexible and lasts for a period of time by balancing Yin and Yang... |
