Network Pharmacology Study And Clinical Curative Effect Observation Of Modified Xiangsha Liujunzi Decoction In The Treatment Of Chronic Renal Failure

Purpose:With the help of network pharmacology research method,this paper analyzes the mechanism of Xiangsha Liujunzi Decoction in the treatment of chronic renal failure;according to the results of network pharmacology analysis,from the perspective of clinical efficacy observation,this paper objectively evaluates the clinical efficacy and safety of Xiangsha Liujunzi Decoction in the treatment of chronic renal failure patients with spleen and kidney deficiency.Material and method: The first part: the network pharmacology research of Xiangsha Liujunzi Decoction in the treatment of chronic renal failure.The active small molecular components and potential targets of Xiangsha Liujunzi Decoction were searched in tcmsp database.Ob ≥ 30% and DL ≥ 0.18 were set as the initial screening conditions.Then the target gene sequence and the corresponding gene name were matched by uiprot database,and the corresponding gene name was added.Five databases including genecards,OMIM,TTD,Pharm GKB and drugbank were used to search for the names of genes related to chronic renal failure.R software was used to draw Wayne map,string database was used to make PPI network,and "active ingredient target gene pathway" network was constructed by Cytoscape software.R software was used to do go function and KEGG enrichment analysis.The second part: the clinical observation of Xiangsha Liujunzi Decoction in the treatment of chronic renal failure.Methods: a retrospective study was conducted to collect 88 patients with chronic renal failure treated by Professor Ma Xiaoyan from September 2018 to August 2020.They were divided into control group(30 cases)and observation group(58 cases).On the basis of control group,they were treated with Xiangsha Liujunzi Decoction for 8 weeks The change data of clinical symptoms of standard and traditional Chinese medicine were sorted out and analyzed.Results:The first part: the network pharmacology research of Xiangsha Liujunzi Decoction in the treatment of chronic renal failure.1.There are 100 kinds of active small molecular components and 790 target genes in Xiangsha Liujunzi Decoction.2.Through topological index analysis,14 important small molecule compounds network nodes were obtained,which were mainly quercetin,luteolin and kaempferol.3.In the disease gene related database,9446 renal failure disease genes and 214 drug and disease intersection targets were retrieved.4.A total of 793 targets and 213 nodes were obtained,including mitogen activated protein kinase 1,nuclear proto oncogene and serine protein kinase.5.Through go function analysis,it was found that the modified Xiangsha Liujunzi Decoction could affect the antioxidant reaction,anti lipopolysaccharide reaction and the reaction of bacterial origin molecules.6.KEGG enrichment analysis showed that the modified Xiangsha Liujunzi Decoction contained 171 potential therapeutic target related pathways such as disease,signal transduction and virus.The second part: the clinical observation of Xiangsha Liujunzi Decoction in the treatment of chronic renal failure.1.compared with before treatment,SCR(P < 0.01)and bun(P < 0.01)in the observation group were significantly decreased,and EGFR(P < 0.01)was significantly increased,with significant statistical difference.SCR(P = < 0.05),bun(P < 0.05)in the control group were lower than before treatment,EGFR(P < 0.05)was higher than before treatment.2.After treatment,SCR(P < 0.05)and bun(P < 0.05)of renal function in the observation group decreased,and EGFR(P < 0.05)increased more than that in the control group,with statistical significance.3.The improvement of syndrome in the observation group was more obvious than that before treatment,and was better than that in the control group,with statistical difference(P < 0.01).4.There were no other adverse drug events in the two groups,and there were no abnormal changes in general vital signs and liver function before and after treatment.It shows that Xiangsha Liujunzi Decoction is suitable,rational,safe and effective.Conclusion:The first part: the network pharmacology research of Xiangsha Liujunzi Decoction in the treatment of chronic renal failure.1.The modified Xiangsha Liujunzi Decoction contains a lot of quercetin,luteolin,kaempferol and other components,which play a role in the treatment of chronic renal failure by affecting the network core targets such as mitogen activated protein kinase 1,nuclear proto oncogene,serine protein kinase,etc;2.Xiangsha Liujunzi Decoction can treat chronic renal failure by regulating the reaction of anti-oxidation and anti lipopolysaccharide,and by influencing disease,signal transduction,antiviral and other related pathways.The second part: the clinical observation of Xiangsha Liujunzi Decoction in the treatment of chronic renal failure.1.Xiangsha Liujunzi Decoction Combined with Haikun Shenxi Capsule can significantly reduce serum creatinine,serum urea nitrogen,improve glomerular filtration rate and protect residual renal function;2.Xiangsha Liujunzi Decoction Combined with Haikun Shenxi Capsule can improve the clinical symptoms of patients,reduce the TCM syndrome score,and the total effective rate is89.66%;3.It is suggested that tonifying the spleen and kidney,strengthening the spleen,removing dampness and blood stasis,dredging the Fu organs and removing turbidity are the three methods of detoxification.
