Application Of Case Management In Community Rehabilitation Of Urban Elderly Patients With Depression

The aging process in China is accelerating,and more and more elderly people suffer from depression.At present,social work practice and academic field pay less attention to senile depression,and their diversified needs are difficult to be met,so they are in an extremely vulnerable position.This paper studies the effect and improvement path of case management in community rehabilitation of urban elderly patients with depression,explores the localization mode of case management,and promotes the development of community rehabilitation of elderly patients with depression in China.The goal of this study is to help the service objects to explore their own advantages,mobilize the surrounding support forces,coordinate social resources,and provide a series of professional social services for them by carrying out cooperation among various social organizations or departments after fully communicating with the service objects and respecting their self-determination through professional and scientific evaluation,diagnosis and analysis of the details of the service objects I will work and serve.In this paper,through the establishment of a case management team,the development of self-management plan,the implementation of psychotherapy,combined with social work case group work methods to continue to carry out services and develop the service object to become a volunteer five processes to implement case management for the service object.In order to change the wrong cognition of the service object,improve self-esteem and self-confidence,enhance the motivation of change;improve the relationship between the service object and family members,help them to obtain family support;guide the service object to participate in the treatment group,help the service object to accept themselves,reconcile with themselves,and reestablish the contact with social members in the group;excavate the potential available resources,so as to enable them to participate in the treatment It's enough to really reintegrate into society.So as to achieve the purpose of physical rehabilitation,psychological rehabilitation and social rehabilitation.The research shows that case management can effectively integrate social resources to meet the multiple needs of service objects in the intervention of community rehabilitation services for urban elderly patients with depression.In the process of service,social workers use case management to help the case owners rationally look at their own problems,strengthen self-awareness,strengthen the rehabilitation motivation of the case owners,improve the psychological state of the case owners,and help them integrate into social life,so as to provide all-round and multi-level services for the case owners.However,the quality of case managers needs to be improved,and the sustainability of case management is difficult to be guaranteed,which restricts the development of case management.It is necessary to further verify whether the case management can be popularized generally.
