Objective:To master the epidemic situation of noise-induced hearing loss(NIHL)and tinnitus in ground crew of PLA Air Force,and analyze the morbidity characteristics and influencing factors of NIHL and tinnitus,so as to provide reference for early diagnosis and treatment of NIHL and protection for occupational noise hazards.Methods:An investigation and analysis of NIHL and tinnitus was conducted through general survey among the ground crew of a certain PLA Air Force by means of questionnaire,scale and clinical audiology examination,so as to grasp the prevalence of NIHL and tinnitus among ground crew and its influencing factors.Results:1.The 356 subjects were male,of which Speech frequency hearing loss(SF-HL),High frequency hearing loss(SF-HL)and Extended high frequency hearing loss(EHF-HL)were 3.1%,23.0% and 55.2%.Working years(OR=2.03),conscious hearing(OR=1.76),usage of HPDs(OR=0.56)and tinnitus(OR=2.40)were the independent influencing factors of HF-HL.Working years(OR=1.69)and usage of HPDs(OR=0.72)were the independent influencing factors of EHF-HL.Audiogram analysis shows that the threshold of extended high frequency increased with the increase of working years,and the prevalence of HF-HL and EHF-HL were significantly higher in the 11-15 years working group and >15 years working group.There was a negative correlation between the pure-tone Audiometry(PTA)threshold and the amplitude of Distortion Product Otoacoustic Emission(DPOAE),and the correlation was highest at similar frequency.With PTA threshold >25 d B HL as hearing loss,diagnostic test analysis showed that the sensitivity,specificity and accuracy of DPOAE results in predicting hearing status declined with the increase of working years.2.In this study,the prevalence of tinnitus was 19.6%.The conscious hearing(OR=1.72),frequency of asking others to repeat words(OR=2.06)and grade of HF-HL(OR=1.57)were independent influencing factors of tinnitus.Age,working years,SAS score and PSQI score in tinnitus group were higher than those in tinnitus group.Tinnitus Handicap Inventory(THI)score was positively correlated with SAS score(rs=0.841)and PSQI score(rs=0.424).The correlation between tinnitus and EHF-HL gradually disappeared with the increase of working years.Conclusions:It can be regarded as the key groups for hearing protection for personnel with working years more than 10 years,low frequency of HPDs,conscious hearing loss,tinnitus,ear blockage,etc.Individuals with tinnitus have higher levels of anxiety and sleep disorders than those without tinnitus,and it is recommended to strengthen psychological counseling and protection against psychological problems for ones with tinnitus.
