Pharmaceutical Care Of Proton Pump Inhibitor Injection Formulation For Surgical Patients In A Hospital

Objective:Based on the existing track record of drug monitoring at home and abroad,we establish pharmacy monitoring of patients with PPI injection surgically used in hospital,prevent stress ulcer(SU),round based on actual use Participate in medical order reviews,prescription reviews,and case analysis Treatment status improves patient treatment planning through case analysis and comparison,summarizes monitoring experience and key points,and improves rationale for PPI use.Pharmacists and doctors work together to break through traditional modes of treatment,deliver individualized doses,compare effects before and after pharmacy monitoring,summarize experience and deficiencies,and help clinicians provide drug and pharmacy services.Lay the foundation for reasonable use.Methods:We obtained PPI cases from hospitals from the Hospital Information System(HIS)in 2018 and developed drug treatment plans for patients using PPI injections in hospitals based on literature data.The patients of 2019 would be carried out preliminary pharmaceutical monitoring and be summarized the medication problems in monitoring.Case analysis is performed on various problems such as drug selection,medication for off-label indications,dosing schedule adjustment,therapeutic drug interactions,therapeutic drug side effects.It will provide the basis for pharmaceutical care.To understand basic patient information such as medical history,allergy history,drug history,and drinking history,evaluate the level of patient care,and perform hierarchical drug care.The main contents of monitoring include pharmacy rounds,medical order reviews,adverse drug reaction monitoring(ADR monitoring),patient monitoring,and discharge education.In addition,we selected two groups of patients which were distinguished before and after drug intervention.In this study,patients who used PPI injections from January to December 2018 were used as a control group,with doctors as the main body,and nurses as an adjunct medical team as a control group for this study.They use their expertise to perform traditional modes of treatment.The intervention group selected patients who used PPI injections from January to December 2019.The team is led by physicians,clinical pharmacists as partners,and nurses as ancillary new medical teams as an intervention group for this study.This team not only has the expertise of doctors,but also participates in the new factor pharmacy monitoring,according to the exclusive plan developed in the previous stage to erform hierarchical monitoring.We organized the information of rational use of drugs,adverse drug reaction,routes of administration,length of hospital stay,cost of patients in hospital and used SPSS 21.0 statistical software for statistical analysis.Statistical results analyze the impact of drug care on the use of PPI injections,whether it is statistically significant(p<0.05).Results:Taking the implementation of pharmaceutical care as a cut-off point,the pharmaceutical care standards for the use of PPI injections by inpatients in our hospital were established according to the problems,drug selection,adverse reactions,drug interactions and other aspects that occurred during the use of PPI injections by inpatients in our hospital.A comparative study found that the intervention of a clinical pharmacist significantly shortened the patient’s medication course,reduced the patient’s non-adapted medications and medication errors,and improved patient satisfaction.Conclusion:This paper establishes the pharmaceutical care standard for inpatients using PPI injections in our hospital,and enables clinical pharmacists to participate in the pharmaceutical care of patients using PPI injections after surgery,which helps clinical pharmacists to train and carry out daily work in the surgical field of our hospital,and lays a foundation for the development of clinical pharmacists.At the same time,the study shows that the medical monitoring program we established can effectively reduce the medication errors caused by wrong medication,reduce the course of disease and incidence of adverse reactions caused by unreasonable medication,and improve patient satisfaction.At the same time,doctors in the surgical field are trained to ensure the safety of drug use and improve the quality of life of patients.
