Study On Risk Factors Related To Progressive Cerebral Infarction With Different TOAST Classification In Qiqihar Area

Abstract/Summary:
Objective: by analyzing the basic information,past history,personal history,biochemical indexes and neuroimaging results of 455 patients with acute cerebral infarction,this study preliminarily explored the risk factors related to progressive cerebral infarction with different TOAST classification,so as to provide some reference value for improving the prognosis of patients.Methods: Patients with acute cerebral infarction hospitalized in the First Affiliated Hospital of Qiqihar Medical College from December 2019 to December 2020 were collected.According to the standard,the patients were divided into progressive cerebral infarction group and non progressive cerebral infarction group.The differences of general conditions,biochemical indexes such as fasting blood glucose and total cholesterol and neuroimaging characteristics such as carotid color Doppler ultrasound were analyzed,and a conclusion was drawn by comparing the data.Results: 1.Among the 455 hospitalized cases,120 patients had progressive cerebral infarction,the incidence rate was 26.4%,and the type of large atherosclerosis accounted for51.7% in the progressive cerebral infarction group.2.Smoking history,neck vascular stenosis > 50%,high total cholesterol,low high-density lipoprotein and high leukocyte count in large-area cerebral infarction group were statistically significant in the comparison between progressive cerebral infarction group and non progressive cerebral infarction group(P < 0.05).3.in the small artery occlusion group,neck vascular stenosis > 50%,large-area cerebral infarction,history of diabetes,high fasting blood glucose and low high-density lipoprotein were statistically significant in the comparison between the progressive cerebral infarction group and the non progressive cerebral infarction group(P <0.05).4.the history of smoking,carotid plaque formation,hypertension,total cholesterol,diabetes and high triglyceride count in the cardiogenic embolism group were statistically significant in the comparison between the progressive cerebral infarction group and the non progressive cerebral infarction group(P < 0.05).5.the history of diabetes,high fasting blood glucose and high total cholesterol count in the other cause cerebral infarction group were statistically significant in the comparison between the progressive cerebral infarction group and the non progressive cerebral infarction group(P < 0.05).6.smoking history,carotid plaque formation,history of diabetes,and high triglyceride count in the unexplained cerebral infarction group were statistically significant in the comparison between the progressive cerebral infarction group and the non progressive cerebral infarction group(P <0.05).7.in the comparison of results between the progressive cerebral infarction group and the non progressive cerebral infarction group,smoking history,history of diabetes,high total cholesterol,carotid plaque formation,large-area cerebral infarction,history of hypertension,cervical vascular stenosis > 50%,low high-density lipoprotein and high fasting blood glucose were statistically significant(P < 0.05).8.the history of diabetes,hypercholesterolemia,carotid plaque,cervical vascular stenosis > 50% and large-area cerebral infarction had statistical significance in the multivariate regression analysis of progressive cerebral infarction group and non progressive cerebral infarction group(P <0.05).Conclusion: 1 Hypercholesterolemia,history of diabetes,cervical vascular stenosis >50%,carotid plaque and large area cerebral infarction were independent risk factors for progressive cerebral infarction.2.Smoking history,massive cerebral infarction,neck vascular stenosis > 50%,high cholesterol,low high density lipoprotein and high leukocyte are the risk factors of progressive cerebral infarction in the group of large atherosclerotic cerebral infarction.In the small artery occlusive cerebral infarction group,large area cerebral infarction,neck vascular stenosis > 50%,history of diabetes,high fasting blood glucose and low high-density lipoprotein were the risk factors for progressive cerebral infarction.Smoking history,carotid plaque formation,history of diabetes,high cholesterol and high triglyceride were the risk factors of progressive cerebral infarction in cardiogenic embolism group.Diabetes history,high fasting blood glucose and high cholesterol were the risk factors of progressive cerebral infarction in other cause cerebral infarction group.Smoking history,carotid plaque formation,history of diabetes mellitus and high triglyceride were the risk factors of progressive cerebral infarction in unexplained cerebral infarction group.
