Background and purpose:With the rapid development of social economy,the improvement of people’s living standards and the ever-changing diet and living habits,cardiovascular disease(CVD)has become a serious public health problem in today’s society.As one of the common CVD,coronary heart disease(CHD)is pathologically based on coronary artery stenosis caused by a series of factors.With the progress of current research,the ratio of platelet to lymphocyte(PLR)and the ratio of triglyceride to high density lipoprotein cholesterol(THR)have attracted more and more attention as a simple and easily available clinical research index.It is expected to become a new index to diagnose coronary heart disease and evaluate the degree of coronary artery stenosis.So in this study,through the basic information of patients in different groups,related hematological indicators,based on Gensini score to evaluate the severity of coronary artery stenosis,to understand the correlation between PLR and THR and coronary heart disease and the degree of coronary artery stenosis.Research methods:A total of 268 patients who underwent coronary angiography in the Department of Cardiology and Cardiac surgery of Qilu Hospital of Shandong University from June 2019 to February 2020 were selected as subjects,including 110 males and 158 females.Baseline clinical data,laboratory test results and coronary artery angiography were collected.imaging results and other indicators.According to the angiography results,the patients were divided into CHD group(147 cases)and non-CHD group(121 cases).According to the Gensini score,patients with coronary stenosis(152 cases)were divided into three groups:mild disease group(72 cases),moderate disease group(69 cases),and severe disease group(11 cases).The main statistical methods used were,parametric test,Mann-Whitney U test,Kruskal-Wallis lway ANOVA test,chi-square test or exact test,logistic regression analysis and the use of receiver operating characteristic curve(ROC)Results:PLR and THR in CHD group were higher than those in non-CHD group,and the difference was statistically significant(p<0.05);Comparison between coronary heart disease group and non-coronary heart disease group.There is no statistical differences in BMI、ALT、AST、GLU、CR、WBC、RBC、PLT、LVEF、Systolic blood pressure and Diastolic pressure(p>0.05).There are statistical differences in Gender、Smoking history、diabetes and Hypertension(p<0.05).The TBIL、DBIL、IBIL、TG、TC、LDL-C and LYM of CHD group were significantly higher than those of non-CHD group(p<0.05).The HDL-C of CHD group were significantly lower than those of non-CHD group(p<0.05).Multivariate Logistic regression analysis showed that age,sex,diabetes,smoking history,hypertension,IBIL,THR and PLR were independent risk factors for CHD.According to Spearman correlation analysis,PLR and THR were positively correlated with Gensini score of coronary artery stenosis(r=0.197,r=0.279,P<0.05).The results of ROC curve analysis showed that when PLR alone was used to predict the incidence of CHD,the AUC was 0.594(p<0.001).When the cutoff value of PLR is 114.21.the diagnostic efficiency is the highest,the sensitivity is 71.43%,and the specificity is 48.76%.When THR alone is used to predict the incidence of CHD,the AUC is 0.684(p<0.001).When the cut-off value of THR is 1.13,the diagnostic efficiency is the highest,the sensitivity is 60.96%,and the specificity is 71.90%.When the multivariate Logistic regression model was used to predict the incidence of CHD,AUC was 0.886(p<0.001).When the probability cut-off value of the model is 0.54,the diagnostic efficiency is the highest,the sensitivity is 78.77%.and the specificity is 80.99%.When PLR alone is used to predict moderate and severe coronary artery disease,the AUC is 0.591(p<0.001).When the cut-off value of PLR is 145.578,the diagnostic efficiency is the highest,the sensitivity is 42.50%,and the specificity is 79.17%.When THR alone was used to predict moderate and severe coronary artery disease,the AUC was 0.639(p<0.001).When the cut-off value of THR was 1.363,the diagnostic efficiency was the highest,the sensitivity was 58.75%,and the specificity was 64.79%.When using multi-factor Logistic model to predict moderate and severe coronary artery disease,the AUC is 0.772(p<0.001).When the cut-off value is 0.675(p<0.001),the diagnostic efficiency is the highest,the sensitivity is 46.15%,and the specificity is 95.77%.Conclusion:1.As a new type of inflammatory markers,PLR and THR are positively correlated with the diagnosis of coronary heart disease and Gensini score.2.PLR and THR can be used as an index to assess the severity of coronary artery disease.3.Age,gender,diabetes,smoking history,hypertension,IBIL,THR and PLR can be used as independent risk factors for CHD.4.Diabetes,TC and LDL-C can be independent risk factors for moderate to severe coronary artery disease. |
