The Study On Quantitative Diagnosis Of Damp Heat Syndrome In Idiopathic Membranous Nephropathy Based On Disease-syndrome Combination

Objective: To establish the TCM syndrome quantitative diagnostic model and diagnostic criteria of damp heat syndrome in idiopathic membranous nephropathy based on disease-syndrome combination,so as to provide a standardized quantitative diagnostic scale for the clinical syndrome differentiation of damp heat syndrome in idiopathic membranous nephropathy.Methods: Through the combination of literature investigation and group discussion,the TCM disease name of idiopathic membranous nephropathy was determined.Take the name of the disease determined by the core group discussion as the key words,extensively collect and sort out the relevant medical records in the ancient and modern literatures,and extract the four diagnosis information from these literatures to form a pool of items.Delphi method was used to collect the opinions of relevant professional TCM clinical experts,and “the first version of the diagnostic scale of damp Heat syndrome in idiopathic membranous nephropathy”was formed.“The first version of the scale”was used as a tool to collect clinical case data and select items with the help of a variety of statistical methods to form “The second version of the scale”.And its reliability and validity were tested.After that,the item weight,threshold and grading interval of the scale were determined to form a diagnostic model and diagnostic criteria.Finally,using it as a tool,a diagnostic test was carried out to test its accuracy.Results: The diagnostic scale of damp heat syndrome in idiopathic membranous nephropathy including 8 items was established.And the reliability and validity of the scale were good.Based on this situation,the quantitative diagnostic standard of damp heat syndrome in idiopathic membranous nephropathy with a diagnostic threshold of 171.5 was established.At the same time,the grading standard of degree was determined.The clinical diagnostic test shows that the diagnostic standard has high sensitivity,specificity and accuracy.Conclusion: The quantitative diagnostic standard of damp heat syndrome in idiopathic membranous nephropathy established through literature investigation,expert questionnaire survey,clinical case data collection,statistical analysis,scale evaluation and other standardized and rigorous steps,which has good maneuverability,accuracy and practicability.The diagnostic criteria will provide a standardized scale for clinical quantitative diagnosis,objective evaluation of curative effect and further study of damp heat syndrome in idiopathic membranous nephropathy.
