| Objective : Collected the relevant information of patients with proximal humeral fractures admitted to the Affiliated Hospital of Shandong University of Traditional Chinese Medicine by retrospective study,and analyzed the morbidity characteristics and the influencing factors of surgical prognosis,so as to provide basis for the prevention,diagnosis and treatment of PHF.And explored the clinical effect of postoperative shoulder function recovery of Shenjin tablet in patients with PHF,so as to provide theoretical evidence for the clinical application of traditional Chinese medicine.Method:1.Through the big data search engines of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine,the clinical data of inpatients with proximal humerus fractures from August 2015 to August 2021 were collected and analyzed for the relationship between the age,gender,BMI,cause of injury,season of onset,tobacco and alcohol history,internal medical diseases,and incidence of PHF so as to summarize its pathogenesis characteristics,osteoporosis to summary the morbidity pattern.2.In patients with PHF who undergo surgical treatment,fracture classification,average cortical thickness of proximal humerus,post-fracture waiting time,operation time,use of traditional Chinese medicine preparations for promoting blood circulation and removing blood stasis,postoperative neck-shaft angle and Neer score were further collected and evaluated to investigate the factors influencing postoperative efficacy.The Neer score of shoulder joint was obtained through follow-up and used as the final evaluation index.They were divided into two groups for case-control study.Using SPSS software and statistical methods,the factors with statistical significance were screened,and the related factors that may affect the functional recovery of shoulder joint after proximal humeral fracture were obtained by logistic regression analysis.3.A total of 60 patients with proximal humerus fractures hospitalized from August2020 to August 2021 were collected and randomly divided into observation group(30cases)and routine group(30 cases).Both groups were treated with open reduction inner fixation with steel plate,and postoperative treatment with conventional medications such as anti-swelling and pain relief and anti-infection.On the basis of conventional treatment,the observation group took 5 Shenjin tablets,tid,and PO for 4 weeks.Observed the postoperative of the two groups’ VAS score and Neer score.Results:1.A total of 734 patients with proximal humerus fractures were collected.The analysis showed that most of the patients were female,the main cause of injury was fall,the age of patients was concentrated between 60 and 80 years old,and the number of patients in summer was significantly less than in other seasons.Among them,661 patients underwent surgery,and 476 patients received traditional Chinese medicine preparations for promoting blood circulation and removing blood stasis during hospitalization.Hypertension,cardiovascular disease,and diabetes are the three major medical complications in patients with PHF.2.A total of 292 patients with PHF were retrospectively selected,and 220 patients hae good Neer scores,with its rate of 75.34%.The univariate analysis showed that there were statistically significant differences(P <0.05)between the good prognosis group and the poor prognosis group in terms of age group,fracture classifications,history of hypertension,history of cerebrovascular disease,postoperative neck-shaft angle,use of traditional Chinese medicine preparations for promoting blood circulation and removing blood stress,functional exercise,BMI,and complications.In addition,the multivariate regression analysis also showed that the difference between fracture classifications,postoperative neck-shaft angle,use of traditional Chinese medicine preparations for promoting blood circulation and removing blood stasis,and functional exercise was statistically significant differences(P <0.05),and those were the influencing factors of postoperative efficacy of PHF.3.The general baseline data of the two groups were comparable(P > 0.05).In the treatment of PHF with Shenjin tablets,compared with the routine group,the observation group was significantly improved in VAS score of 1 week and 4 weeks after operation and Neer score at 3 months after operation with statistically significant differences(P < 0.05).There were no serious adverse reactions in the two groups.Conclusion : Proximal humerus fractures are most common in elderly women.Hypertension,cardiovascular disease and diabetes are the three major medical complications in patients with PHF.Fracture classifications,postoperative neck-shaft angle,use of traditional Chinese medicine preparations for promoting blood circulation and removing blood stress,and functional exercise are the independent influencing factors of postoperative efficacy of PHF.In the treatment of proximal humeral fractures,shenjin tablet can effectively relieve the patients’ pain and improve their shoulder joint function with good curative effect. |
