Clinical Observation Of Yanghe Decoction In The Treatment Of Helcosis Of Granulomatous Mastitis And The Effect On Immune Inflammatory Response

Objective: To summarize the clinical effects of Yanghe Decoction by analyzing the TCM syndromes and local lesion changes in patients with helcosis of granulomatous mastitis after Yanghe Decoction treatment;To explore the mechanism of action of Yanghe Decoction in the treatment of helcosis of granulomatous mastitis by contrasting the levels of immune inflammatory factors in the serum of patients before and after treatment.Methods: A total of 60 patients with helcosis of granulomatous mastitis who were hospitalized in the outpatient clinic or ward of breast thyroid surgery,Affiliated Hospital of Shandong University of Chinese medicine,from November 2020 to February 2022,were screened,and the patients were randomly divided into treatment and control groups in a 1:1 ratio,with 30 patients in each group.The affected breast was debrided and replaced in both the treatment and control groups,and the treatment group received Yanghe Decoction based on this formula.(1)The clinical efficacy of Yanghe Decoction in the treatment of helcosis of granulomatous mastitis was evaluated by a comprehensive analysis comparing the wound area,lesion range under color ultrasound,syndrome improvement rate of traditional Chinese medicine(TCM),total effective rate,and adverse events between the two groups before and after treatment.(2)The effects of Yanghe Decoction on immune inflammatory factors were analyzed by detecting the levels of IL6,IL17 A,and TNF-a in the serum of the two patients before and after treatment.(3)Telephone follow-up was conducted 2 months after the end of treatment,and the recurrence rate was statistically analyzed.All data were analyzed by SPSS 26 software for analysis.Results:(1)The treatment group was superior to the control group in terms of wound area reduction rate,lesion extent reduction rate under color ultrasound,TCM syndrome improvement rate,and total effective rate,with significant statistical difference(P < 0.05)between the two groups,and the total effective rate of the treatment group reached 96.67%.(2)The serum levels of IL6,IL17 A,and TNF-a in the treatment group were decreased and better than those in the control group,with statistically significant differences between the two data sets(P < 0.05).(3)At the follow-up,the recurrence rate was lower in the treatment group versus the control group,and there was no statistical significance between the two data sets(P >0.05).No adverse effects occurred in either group of patients during the course of the experimental study.Conclusions: Yanghe Decoction have significant efficacy in the treatment of helcosis of granulomatous mastitis,are safe to use,improve the clinical syndrome,and accelerate disease healing;The mechanism of action may be related to Yanghe decoction reducing the levels of inflammatory factors in the body;The follow-up shows that the recurrence rate of taking Yanghe Decoction is low and can be applied in the clinic.
