Comparis On Of The Efficacy Of Recombinant Human Brain Natriuretic Peptide Combined With Milrinone And Milrinone In The Treatment Of Acute Left Heart Failure

Objective:To explore and compare the clinical efficacy of recombinant human brain natriuretic peptide combined with milrinone and milrinone in the treatment of acute left heart failure,and to provide more theory and experience for the treatment of acute left heart failure.Methods:Collect the clinical data of 98 patients with acute left heart failure admitted to Jilin Province People’s Hospital from January 2016 to October 2021;The natriuretic peptide combined with milrinone group)and the control group（milrinone group）,49 cases in each of the observation group and the control group.Observation group:Recombinant human brain natriuretic peptide combined with milrinone was given on the basis of conventional treatment for 7 days,and then changed to conventional treatment after 7 days.Control group:On the basis of conventional treatment,milrinone was given treatment for 7 days,and then changed to conventional treatment.Analyze the two groups of LVEF,NT-pro BNP,CRP,dyspnea score and systemic symptom score,vital signs,urine output,PH,PaO₂,PaCO₂,renal function,liver function,electrolytes and other indicators,and compare the clinical treatment of the two groups before and after treatment.Efficiency and incidence of adverse reactions.Results:1)Clinical efficacy:After treatment,the total effective rate of the observation group was89.8%and that of the control group was 75.5%,P<0.05,the difference was obvious,and the observation group had better efficacy.2)LVEF,NT-pro BNP,CRP,dyspnea score,and systemic symptom score:After treatment,LVEF in both groups increased,while NT-pro BNP,CRP,dyspnea score,and systemic symptom score decreased.The improvement was even greater,P<0.05,the difference was statistically significant.3)Vital signs,urine output,PaO₂:After treatment,the heart rate of the two groups improved,the systolic blood pressure decreased,the urine output increased,and PaO₂increased.Except for the heart rate,other indicators were tested,and all were P<0.05,the difference was Statistical significance;the observation group improved more obviously.4)Safety:Before and after treatment,the values of PH,PaCO₂,BUN,SCr,AST,ALT,K⁺,and Na⁺were compared within and between groups.According to statistics,P>0.05,no significant difference.Adverse reactions:The incidence of adverse reactions in the two groups were 6.1%and 8.2%,P>0.05,the difference was not statistically significant.Conclusion:For the treatment of acute left heart failure,the combination of recombinant human brain natriuretic peptide combined with milrinone can improve the symptoms,signs and auxiliary examination indicators of patients more obviously,and the clinical effect is better.Liver and kidney function,electrolytes,blood carbon dioxide partial pressure and blood p H have no obvious influence,the incidence of adverse reactions is low,and the safety is high.It is worth promoting.
